摘　　要：目的:探讨密网支架辅助下介入栓塞术治疗颅内宽颈大动脉瘤(IWNA)的临床效果。方法:选取2018年1月—2022年12月黔南布依族苗族自治州人民医院收治的IWNA患者70例作为研究对象,根据随机数字表法将其分成对照组(单纯弹簧圈栓塞术治疗,n=35)、观察组(密网支架辅助弹簧圈栓塞术治疗,n=35)。对比两组栓塞情况、影像学指标(动脉瘤直径、瘤颈)、术后并发症与复发情况。结果:观察组完全致密栓塞率高于对照组(P=0.022)。术前,两组动脉瘤直径、瘤颈对比,无统计学差异(P>0.05);术后7 d,两组动脉瘤直径、瘤颈缩短,观察组短于对照组(P<0.001)。观察组术后并发症发生率、复发率低于对照组(P<0.05)。结论:采用密网支架辅助介入栓塞术治疗IWNA患者可提高完全栓塞率,促进动脉瘤直径、瘤颈缩小,降低并发症发生率、复发率。Objective:To investigate the clinical effect of interventional embolization assisted by dense mesh stent in the treatment of large intracranial wide-necked aneurysm(IWNA).Methods:A total of seventy IWNA patients admitted to Qiannan Buyi and Miao Autonomous Prefecture People's Hospital from January 2018 to December 2022 were selected as the study objects.They were divided into control group(embolization with spring coil alone,n=35)and observation group(embolization with close mesh stent assisted,n=35)according to random number table method.Embolization,imaging indexes(aneurysm diameter,tumor neck),postoperative complications and recurrence were compared between the two groups.Results:The rate of complete dense embolization in observation group was higher than that in control group(P=0.022).Before operation,there were no significant differences in aneurysm diameter and neck between the two groups(P>0.05);on the 7th day after operation,the diameter and neck of aneurysm in both groups were shortened,and the observation group was shorter than the control group(P<0.001).The incidence of postoperative complications and recurrence in the observation group were lower than those in the control group(P<0.05).Conclusion:The close mesh stent assisted interventional embolization in the treatment of IWNA patients can increase the complete embolization rate,promote the reduction of aneurysm diameter and tumor neck,and reduce the incidence of complications and recurrence.
