窝沟封闭术联合氟保护漆对小儿龋齿的预防效果

摘　　要：目的:分析窝沟封闭术辅以氟保护漆预防小儿龋齿的效果。方法:选取2020年1月—2022年1月在莒南县洙边镇卫生院进行口腔检查的儿童88例(恒牙数量350颗)为研究对象,采用随机数字表法分为研究组与对照组,各44例。对照组采取窝沟封闭术进行龋齿预防,研究组采取窝沟封闭术辅以氟保护漆进行龋齿预防。比较两组封闭剂脱落情况、龋齿发生情况、龈沟液中的炎性因子水平及咀嚼功能。结果:研究组封闭剂总脱落率低于对照组(P<0.001);研究组龋齿发生率低于对照组(P=0.002);手术后6个月,两组龈沟液分泌蛋白-1、巨噬细胞炎症蛋白-1α水平均降低,研究组低于对照组(P<0.05);手术后6个月,两组咀嚼力度、咀嚼面积、咀嚼次数评分均升高,研究组高于对照组(P<0.05)。结论:窝沟封闭术辅以氟保护漆预防小儿龋齿的效果比单一窝沟封闭术更好,封闭剂保留度更高,可以降低龋齿发生率、龈沟液炎性因子水平,改善咀嚼功能。Objective:To analyze the effect of pit and fissure sealant supplemented with fluoride protective varnish on the prevention of pediatric dental caries.Methods:Eighty-eight cases of children(number of permanent teeth 350)who underwent oral examination in the Junan County Zhubian Town Health Hospital from January 2020 to January 2022 were selected for the study,and were divided into a study group and a control group,each of which consisted of forty-four cases,using the method of randomized numerical table.In the control group,caries prevention was carried out by pit and fissure sealant,and in the study group,caries prevention was carried out by pit and fissure sealant supplemented with fluoride protective varnish.The two groups were compared in terms of sealant shedding,caries occurrence,inflammatory factors levels in gingival sulcus fluid and masticatory function.Results:The total rate of sealant shedding in the study group was lower than that in the control group(P<0.001);the incidence of dental caries in the study group was lower than that in the control group(P=0.002);6 months after surgery,the levels of secretory protein-1 and macrophage inflammatory protein-1αin the gingival sulcus fluid in the two groups were reduced,and the study group was lower than that in the control group(P<0.05);six months after the surgery,chewing strength,chewing area,and number of chewing scores were elevated in both groups,and the study group was higher than the control group(P<0.05).Conclusion:Pit and fissure sealant supplemented with fluoride protective varnish is more effective than single pit and fissure sealant in preventing pediatric caries,with higher retention of sealant,which can reduce the incidence of caries,the levels of inflammatory factors in gingival sulcus fluid,and improve the function of mastication.
