摘 要:目的:探讨心脏超声检查联合血清心肌标志物检测在乳腺癌化疗患者心脏不良反应风险评估中的应用效果。方法:回顾性分析2022年1—12月临沂市中心医院收治的120例乳腺癌化疗患者的临床资料,根据是否发生心脏不良反应分为无心脏毒性组(n=78)和心脏毒性组(n=42)。比较两组心脏超声指标、血清心肌标志物,分析心脏超声检查联合血清心肌标志物检测对乳腺癌化疗患者心脏不良反应的预测价值。结果:心脏毒性组左室射血分数水平低于无心脏毒性组,左室收缩末期容积、舒张期室间隔厚度高于无心脏毒性组(P<0.001)。心脏毒性组血清心肌肌钙蛋白T、肌酸激酶同工酶水平高于无心脏毒性组(P<0.001)。心脏超声检查联合血清心肌标志物预测乳腺癌化疗患者心脏不良反应发生的曲线下面积为0.947,灵敏度为90.8%,特异度为94.5%,均高于单项指标检测。结论:心脏超声检查联合血清心肌标志物检测评估乳腺癌化疗患者心脏不良反应风险的价值较高,可为早期心脏保护性方案的制定提供有效参考。Objective:To investigate the application effect of cardiac ultrasonography combined with detection of serum myocardial markers in the risk assessment of cardiac adverse reactions in breast cancer patients undergoing chemotherapy.Methods:The clinical data of one hundred and twenty breast cancer patients treated by chemotherapy in Linyi Central Hospital from January to December 2022 were retrospectively analyzed.According to the occurrence of cardiac adverse reactions,they were divided into no cardiotoxic group(n=78)and cardiotoxic group(n=42).Cardiac ultrasound indexes and serum myocardial markers were compared between the two groups.The predictive value of cardiac ultrasonography combined with detection of serum myocardial markers in breast cancer patients with chemotherapy were analyzed.Results:The left ventricular ejection fraction in the cardiotoxic group was lower than that in the no cardiotoxic group,and the left ventricular end-systolic volume and diastolic septal thickness were higher than those in the no cardiotoxic group(P<0.001).Serum troponin T and creatine kinase isoenzyme levels in cardiotoxic group were higher than those in no cardiotoxic group(P<0.001).The area under the curve of cardiac ultrasound combined with serum myocardial markers to predict the occurrence of adverse cardiac reactions in patients with breast cancer undergoing chemotherapy was 0.947,with sensitivity of 90.8%and specificity of 94.5%,which were higher than the detection of single index.Conclusion:Cardiac ultrasonography combined with serum myocardial marker detection has a high value in assessing the risk of cardiac adverse reactions in patients with breast cancer chemotherapy,which can provide an effective reference for the formulation of early cardiac protective protocols.
