多层螺旋CT在胃腺癌及瘤周组织浆膜浸润评估中的应用

摘　　要：目的探讨多层螺旋CT(MSCT)在胃腺癌及瘤周组织浆膜浸润评估中的作用。方法纳入2021年1月至2023年4月期间98例经病理证实的胃腺癌患者作为观察组,另选取同时期50例健康志愿者作为对照组,均接受MSCT扫描,胃腺癌患者均行手术治疗并获取病理报告,以病理诊断为“金标准”,判断MSCT在胃腺癌诊断及瘤周组织浆膜浸润评估中的价值。结果观察组98例胃腺癌患者中,MSCT成功识别出93例,5例未能识别,对照组50例健康志愿者中,MSCT成功排除48例,误诊2例,MSCT诊断胃腺癌的敏感度、特异度、准确度分别为94.90%、96.00%、95.27%,与病理诊断一致性检验Kappa值为0.90;MSCT识别瘤周组织浆膜浸润的敏感度、特异度、准确度分别为87.50%、90.00%、88.78%,与病理诊断一致性检验Kappa值为0.78,均高于术中诊断;MSCT识别浅层浆膜浸润的敏感度、特异度、准确度分别为83.33%、88.37%、86.21%,识别深层浆膜浸润的敏感度、特异度、准确度分别为90.00%、100.00%、94.12%,与病理诊断一致性检验Kappa值分别为0.75、0.87,均高于术中诊断。结论MSCT在胃腺癌及瘤周组织浆膜浸润评估中具有一定应用价值。Objective To explore the role of multi-slice spiral CT(MSCT)in the evaluation of gastric adenocarcinoma and peritumoral tissue serosa infiltration.Methods 98 patients with pathologically confirmed gastric adenocarcinoma from January 2021 to April 2023 were included as observation group,and 50 healthy volunteers during the same period were enrolled as control group,and they all received MSCT scan.All patients with gastric adenocarcinoma underwent surgical treatment and obtained pathological report.Pathological diagnosis was taken as the gold standard to judge the value of MSCT on the evaluation of gastric adenocarcinoma and peritumoral tissue serosa infiltration.Results Among 98 patients with gastric adenocarcinoma in observation group,93 cases were successfully identified by MSCT and 5 cases were not identified.Among 50 healthy volunteers in control group,48 cases were successfully excluded by MSCT and 2 cases were misdiagnosed.The sensitivity,specificity and accuracy of MSCT in diagnosing gastric adenocarcinoma were 94.90%,96.00%and 95.27%respectively,and the Kappa value of consistency test with pathological diagnosis was 0.90.The sensitivity,specificity and accuracy of MSCT in identifying peritumoral tissue serosa infiltration were 87.50%,90.00%and 88.78%,and the Kappa value of consistency test with pathological diagnosis was 0.78,which was higher than that of intraoperative diagnosis.The sensitivity,specificity and accuracy of MSCT were 83.33%,88.37%and 86.21%in identifying shallow serosa infiltration and were 90.00%,100.00%and 94.12%in identifying deep serosa infiltration,and the Kappa values of consistency test with pathological diagnosis were 0.75 and 0.87 respectively,which were higher than those of intraoperative diagnosis.Conclusion MSCT has a certain application value on the evaluation of gastric adenocarcinoma and peritumoral tissue serosa infiltration.
