摘　　要：脑小血管病(CSVD)是一种进展缓慢的疾病,可引起患者步态异常、吞咽困难、膀胱功能紊乱,甚至抑郁、痴呆等症状,当病变逐渐进展,可严重影响患者的生活质量。高血压、遗传缺陷等引起的血管内皮损伤是其目前比较明确的病因。虽然CSVD通常主要涉及脑小动脉的病理变化,但近年来人们也逐渐重视脑实质中的小静脉,特别是负责引流深层脑白质血液的深髓静脉在CSVD中的作用。深髓静脉的改变可能参与CSVD的病理过程,同时可能在CSVD认知障碍中扮演着重要角色。本文主要综述了深髓静脉与CSVD的研究进展,以期为预防CSVD提供理论依据。Cerebral small vessel disease(CSVD)is a slowly progressive disease that can lead to abnormal gait,dysphagia,urinary dysfunction,and even symptoms such as depression and dementia.As the disease progresses,it significantly impacts the patient's quality of life.Vascular endothelial damage caused by conditions like hypertension and genetic defects is a well-established cause of CSVD.Although CSVD usually involves pathological changes mainly in the small cerebral arterial,in recent years there has also been growing attention to the effect of small veins within brain parenchyma,particularly deep cerebral veins responsible for draining deep white matter on CSVD.Alterations in deep medullary veins may be involved in the pathological process of CSVD and may also play an important role in CSVD cognitive impairment.This article mainly reviews the research progress of deep medullary veins and CSVD to provide theoretical foundations for preventing CSVD.
