多层螺旋CT联合血清CA199、CA242、CEA对胆管癌的诊断效能

摘　　要：目的研究多层螺旋CT联合血清糖类抗原199(CA199)、糖类抗原242(CA242)、癌胚抗原(CEA)对胆管癌的诊断效能。方法将医院2018年1月至2023年1月收治的81例胆管癌患者作为胆管癌组,另取同期收治的80例良性胆道疾病患者作为对照组。两组患者均开展多层螺旋CT检查以及血清CA199、CA242、CEA水平检测。比较两组的血清CA199、CA242、CEA水平;将病理检查结果为金标准,分析并比较多层螺旋CT与血清CA199、CA242、CEA单独及联合检测对胆管癌的诊断效能;比较不同肿瘤T分期的胆管癌患者血清CA199、CA242、CEA水平。结果胆管癌组的血清CA199、CA242、CEA水平高于对照组,差异具有统计学意义(P<0.05)。多层螺旋CT联合血清CA199、CA242、CEA诊断胆管癌的灵敏度、特异度及准确度高于多层螺旋CT与血清CA199、CA242、CEA单独诊断,差异具有统计学意义(P<0.05)。T_(3)+T_(4)期胆管癌患者的血清CA199、CA242、CEA水平高于T_(1)+T_(2)期患者,差异具有统计学意义(P<0.05)。结论多层螺旋CT联合血清CA199、CA242、CEA诊断胆管癌的效能较佳,值得推广。Objective To study the diagnostic efficacy of multi-slice spiral CT combined with serum carbohydrate antigen 199(CA199),carbohydrate antigen 242(CA242)and carcinoembryonic antigen(CEA)for cholangiocarcinoma.Methods A total of 81 patients with cholangiocarcinoma admitted in the hospital from January 2018 to January 2023 were selected as cholangiocarcinoma group,and 80 patients with benign biliary tract diseases admitted in the hospital during the same period were selected as control group.Multi-slice spiral CT examination and serum CA199,CA242 and CEA levels detection were performed in both groups.The levels of serum CA199,CA242 and CEA were compared between the two groups;the result of pathological examination was taken as the gold standard,and the diagnostic efficacy of multi-slice spiral CT and serum CA199,CA242,CEA alone and combined detection for cholangiocarcinoma was analyzed and compared;the levels of serum CA199,CA242 and CEA in patients with cholangiocarcinoma with different tumor T stages were compared.Results The levels of serum CA199,CA242 and CEA in the cholangiocarcinoma group were higher than those in the control group,and the differences were statistically significant(P<0.05).The sensitivity,specificity and accuracy of multi-slice spiral CT combined with serum CA199,CA242 and CEA in the diagnosis of cholangiocarcinoma were higher than those of multi-slice spiral CT and serum CA199,CA242 and CEA alone diagnosis,and the differences were statistically significant(P<0.05).The levels of serum CA199,CA242 and CEA in patients with stage T_(3)+T_(4)cholangiocarcinoma were higher than those in patients with stage T_(1)+T_(2),and the differences were statistically significant(P<0.05).Conclusion The multi-slice spiral CT combined with serum CA199,CA242 and CEA in the diagnosis of cholangiocarcinoma has a better efficacy,which is worthy of promotion.
