经脐单孔腹腔镜手术与经阴道自然腔道内镜手术治疗卵巢良性肿瘤的效果比较

摘　　要：目的探究经脐单孔腹腔镜手术与经阴道自然腔道内镜手术治疗卵巢良性肿瘤的临床效果。方法队列研究。抽取2020年5月至2023年5月汉中市人民医院收治的卵巢良性肿瘤患者100例,按随机数字表法分为对照组和观察组,每组50例。对照组采用经脐单孔腹腔镜手术,观察组采用经阴道自然腔道内镜手术。比较两组手术相关指标(手术时间、术中出血量、术后排气时间、住院时间)、24 h氧化应激指标[丙二醛(MDA)、超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-Px)]及并发症发生情况。结果观察组术后排气和住院时间均短于对照组(P均<0.05),术中出血量少于对照组(P<0.05)。术后,观察组MDA水平低于对照组,SOD和GSH-Px水平均高于对照组(P均<0.05)。观察组并发症发生率(2.00%,1/50)低于对照组(14.00%,7/50),P<0.05。结论卵巢良性肿瘤患者采用经阴道自然腔道内镜手术可缩短手术时间,减少术中出血量,促进术后恢复,降低氧化应激反应及并发症发生率。ObjectiveTo investigate the clinical effect of transumbilical single-port laparoscopic surgery and transvaginal natural-orifice transluminal endoscopic surgery in the treatment of benign ovarian tumors.MethodsA total of 100 patients with benign ovarian tumors treated in Hanzhong People’s Hospital from May 2020 to May 2023 were selected for the cohort study,and they were divided into a control group and an observation group by random number table method,with 50 cases in each group.The control group was treated by transumbilical single-port laparoscopic surgery,while the observation group was treated by transvaginal natural-orifice transluminal endoscopic surgery.The surgical related indicators,including operation duration,intraoperative blood loss,postoperative exhaust time and hospital stay,24-hour oxidative stress indicators,including malondialdehyde(MDA),superoxide dismutase(SOD)and glutathione peroxidase(GSH-Px),and incidence of complications were compared between the two groups.ResultsThe postoperative exhaust time and hospital stay in the observation group were shorter than those in the control group(all P<0.05).The intraoperative bleeding volume in the observation group was less than that in the control group(P<0.05).After operation,the level of MDA in the observation group was lower than that in the control group,while the levels of SOD and GSH-Px were higher than those in the control group(all P<0.05).The incidence of complications in the observation group(2.00%,1/50)was lower than that in the control group(14.00%,7/50),P<0.05.ConclusionsTransvaginal natural-orifce transluminal endoscopic surgery in the treatment of benign ovarian tumors can shorten the operation duration,reduce intraoperative bleeding volume,promote postoperative recovery,and decrease the incidences of oxidative stress reactions and complications.
