胶原蛋白海绵联合浓缩生长因子充填根尖囊肿的临床研究

摘　　要：目的探讨胶原蛋白海绵联合浓缩生长因子填充根尖囊肿术后骨缺损的疗效。方法将需要进行手术治疗的根尖囊肿患者随机分为试验组和对照组(各30例),进行囊肿刮除手术后,试验组将浸润浓缩生长因子(CGF)渗出液的胶原蛋白海绵填入术后遗留的骨腔,表面覆盖CGF膜,常规缝合;对照组术后骨腔不填CGF和胶原蛋白海绵,常规止血缝合。测量并记录术前、术后肿胀度和张口度,术后疼痛指数。分别于术后即刻、术后1、3、6个月拍摄锥形束CT(CBCT),同期测量术区角化龈宽度,比较2组手术前后CBCT和临床数据的变化。结果术后3个月,2组牙槽骨缺损和角化龈宽度差异有统计学意义(P<0.05)。术后6个月,试验组与对照组的角化龈宽度差异有统计学意义(P<0.05)。2组术后肿胀度和疼痛指数比较,差异均有统计学意义(P<0.05)。结论在根尖囊肿术后遗留的骨腔中填入胶原蛋白海绵和CGF,可以减轻术后肿胀、疼痛等不良反应,对促进骨愈合和减少附着龈的丢失具有一定的临床意义。Objective To investigate the effect of collagen sponge combined with concentrated growth factor on bone defects after apical cyst surgery.Methods A total of 60 patients were randomly divided into the experimental group and the control group(30 cases each).Collagen sponge infiltrated with concentrated growth factor(CGF)exudate was filled into the bone cavity remaining of experimental group,and the surface was covered with CGF membrane,no filling was placed in the control group.The degree of postoperative swelling,mouth opening degree and pain index were measured and recorded before and after operation.Cone beam computed tomography(CBCT)was performed at immediately,and 1、3、6 months post-surgery,and the keratinized gingiva width was measured at immediately,and 1、3、6 months post-surgery.The changes of clinical and CBCT data were compared between two groups.Results Three months after operation,there were significant differences between the two groups in the decreased alveolar bone and the keratinized gingiva width(P<0.05).And six months after operation,there was significant differences in the keratinized gingiva width between the the experimental group and the control group(P<0.05).There were significant differences between the two groups in postoperative swelling and pain index(P<0.05).Conclusion The occurrence of postoperative swelling,pain and other adverse reactions could be reduced by implanting medical collagen and concentrated growth factor in the bone cavity,which can promote osteogenesis and reduce the absorption of the attached gum.
