摘 要:目的比较配对氨基酸转换酶4(paired amino acids converting enzyme 4,PACE4)亚型PACE4-FL和PACE4-CT在良性和恶性甲状腺结节中的表达分布情况,为其作为鉴别甲状腺良恶性结节生物学标志物提供参考数据。方法查找鞍山市肿瘤医院2017—2020年接受甲状腺切除术的患者的甲状腺组织块。根据病理分型对其进行分类,恶性结节选择乳头状癌、滤泡癌和髓样癌。良性结节,选择增生型、胶质、腺瘤型结节、滤泡腺瘤以及甲状腺炎类型。使用PACE4的亚型-全长PACE4蛋白即PACE4-FL及其替代亚型PACE4-CT抗体对相应的组织切片进行免疫染色。对肿瘤病变区域和其周围的正常组织进行比较,由有经验的病理医生对染色情况进行评估分析。结果非病变甲状腺组织不表达或低表达PACE4-FL,差异有统计学意义(P<0.01)。恶性结节组PACE4-FL表达率明显高于良性结节组,差异有统计学意义(P<0.01),PACE4-FL免疫染色特异性为95.8%。在恶性病变区域,PACE4-FL在典型的乳头状癌变中表达最明显。与此相反,在滤泡型乳头状癌中均呈低表达。在滤泡癌中,60%显示免疫染色高表达。良性结节中,PACE4-CT在病变和非病变(正常)的组织中表达大体相同,差异无统计学意义(P>0.05)。而在恶性结节中,PACE4-CT在病变区域中的表达明显低于相邻非病变区域。结论PACE4-FL和PACE4-CT在甲状腺恶性肿瘤中高、低差异性表达,尤其是PACE4-FL的差异性表达有望成为检测或筛选甲状腺癌的一个潜在生物学标志物。Objective To compare the expression distribution of paired amino acids converting enzyme 4(PACE4)subtypes PACE4-FL and PACE4-CT in benign and malignant thyroid nodules,providing reference data for the identification of benign and malignant thyroid nodules.Methods Thyroid tissue chunks from patients who underwent thyroidectomy in Anshan Cancer Hospital from 2017 to 2020 was found.According to pathological types,the malignant nodules were selected as papillary carcinoma,follicular carcinoma and medullary carcinoma.Benign nodules,proliferative,glial,adenomatous nodules,follicular adenoma and thyroiditis types were selected.The corresponding tissue sections were immunostained with antibodies to PACE4-FL,the full-length PACE4 protein subtype of PACE4,and its replacement subtype PACE4-CT.The lesion area of the tumor was compared with the surrounding normal tissue,and the staining was evaluated by an experienced pathologist.Results There was no or low expression of PACE4-FL in non-diseased thyroid tissues(P<0.01).The expression rate of PACE4-FL in malignant nodules was significantly higher than that in benign nodules(P<0.01),and the immunostaining specificity of PACE4-FL was 95.8%.In malignant lesions,PACE4-FL was most clearly expressed in typical papillary carcinomas.On the contrary,the expression was low in follicular papillary carcinoma.In follicular carcinoma,60% showed high expression of immunostaining.In benign nodules,the expression of PACE4-CT was similar in pathological and non-pathological(normal)tissues(P>0.05).In malignant nodules,the expression of PACE4-CT in the lesion area was significantly lower than that in the adjacent non-lesion area.Conclusion The differential expression of PACE4-FL and PACE4-CT in thyroid cancer is high and low,especially the differential expression of PACE4-FL is expected to be a potential biomarker for the detection or screening of thyroid cancer.
