摘  要:目的比较基于Scheimpflug成像原理的Pentacam AXL和基于扫频源OCT(SS-OCT)原理的CASIA 2在近视患者前房参数测量中的差异性、相关性及一致性。设计诊断技术评价。研究对象2021年12月-2022年12月就诊于北京同仁医院眼科屈光中心门诊的近视患者50例(50眼),平均年龄(29.62±6.79)岁,将右眼纳入分析。方法使用Pentacam AXL和CASIA 2测量患者的前房深度ACD(Epi)(中央角膜前表面至晶状体前表面的距离)、ACD(Endo)(中央角膜后表面至晶状体前表面的距离)、前房容积(ACV)和前房角(ACA)。采用配对t检验、Pearson相关性分析及Bland-Altman分析评价两种仪器测量患者右眼的数据间的差异性、相关性及一致性。主要指标ACD(Epi)、ACD(Endo)、ACV、ACA,Bland-Altman分析中的95%一致性界限(LoA)和95%LoA范围内点[n(%)]。结果Pentacam AXL测量的ACD(Epi)、ACD(Endo)、ACV、ACA分别为(3.60±0.22)mm、(3.09±0.22)mm、(175.48±23.53)mm^(3)、(40.27±5.30)°;CASIA 2测量的ACD(Epi)、ACD(Endo)、ACV、ACA分别为(3.66±0.21)mm、(3.15±0.21)mm、(175.97±21.58)mm^(3)、(47.04±8.66)°。与Pentacam AXL相比,CASIA 2测得的ACD(Epi)、ACD(Endo)及ACA均较大(t=8.04、7.67、5.25,P均<0.001),而ACV两者无统计学差异(t=0.53,P=0.60)。两仪器在ACD(Epi)、ACD(Endo)及ACV均呈显著正相关(r=0.969、0.964、0.961,P均<0.001);ACA无明显相关性(r=0.216,P=0.132)。两仪器在ACD(Epi)、ACD(Endo)、ACV、ACA的95%LoA分别为(-0.04~0.17)mm、(-0.05~0.18)mm、(-12.49~13.47)mm^(3)、(-11.11~24.65)°。位于95%LoA范围内的点分别为48(96%)、48(96%)、50(100%)、45(90%)。结论Pentacam AXL与CASIA 2在前房参数测量中,ACD虽然有较小差异,但相关性及一致性较好,结果可以互换使用;ACV无显著差异,相关性及一致性较好,结果可以互换使用;ACA有显著差异,无明显相关性,一致性较差,结果不建议互换使用。Objective To compare the difference,correlation and agreement of anterior chamber biological parameters measured by Scheimpflug tomography Pentacam AXL and swept-source anterior segment optical coherence tomography CASIA 2 in myopic eyes.Design Diagnostic technique evaluation.Participants 50 myopic patients(50 eyes)of Beijing Tongren Hospital from December 2021 to December 2022,with an average age of(29.62±6.79)years.The data of the right eye was included for analysis.Method Pentacam AXL and CASIA 2 were used to measure anterior chamber depth(ACD),including ACD(Epi),which means the distance from the corneal epithelium to the anterior surface of the lens,and ACD(Endo),which means the distance from the corneal endothelium to the anterior surface of the lens,anterior chamber volume(ACV)and anterior chamber angle(ACA).Paired t-test,Pearson correlation analysis and Bland-Altman analysis were used to evaluate the differences,correlation and agreement between the two measurements.Main Outcome Measures The value of ACD(Epi),ACD(Endo),ACV,ACA,95%limits of agreement(LoA)and the probability of points inside the boundary line of 95%LoA in Bland-Altman analysis.Results ACD(Epi),ACD(Endo),ACV and ACA measured by Pentacam AXL were(3.60±0.22)mm,(3.09±0.22)mm,(175.48±23.53)mm^(3) and(40.27±5.30)°,respectively.ACD(Epi),ACD(Endo),ACV and ACA measured by CASIA 2 were(3.66±0.21)mm,(3.15±0.21)mm,(175.97±21.58)mm^(3) and(47.04±8.66)°,respectively.Compared with the values in Pentacam AXL,the ACD(Epi),ACD(Endo)and ACA were larger in CASIA 2 with significant differences(t=8.04,7.67,5.25,all P<0.001).There was no significant difference in ACV(t=0.53,P=0.60).The correlation of ACD(Epi)(r=0.969,P<0.001),ACD(Endo)(r=0.964,P<0.001)and ACV(r=0.961,P<0.001)were significant.There was no significant correlation of ACA(r=0.216,P=0.132)between Pentacam AXL and CASIA 2.The Bland Atman analysis showed that the 95%LoA of ACD(Epi),ACD(Endo),ACV and ACA were(-0.04~0.17)mm,(-0.05~0.18)mm,(-12.49~13.47)mm^(3) and(-11.11~24.65)°.The probability
