摘 要:目的探讨基于时间护理的呼吸训练联合排痰管理在肺癌患者围手术期的干预效果。方法选取2021年1月至2023年8月九江市第三人民医院收治的60例肺癌手术患者作为研究对象,按照随机数字表法分为对照组(30例)与研究组(30例)。对照组采用常规护理,研究组采用基于时间护理的呼吸训练、排痰管理。比较两组的自我管理能力评分、肺功能指标、术后并发症发生率、术后疼痛评分、术后恢复时间。结果护理后,研究组的自我管理能力各项分数高于对照组,差异有统计学意义(P<0.05)。护理后,研究组的1秒用力呼吸容积(FEV1)、1秒用力呼吸容积与肺活量比值(FEV1/FVC)均高于对照组,差异有统计学意义(P<0.05)。研究组的术后并发症总发生率、疼痛评分均低于对照组,研究组在手术后的首次离床活动时间、住院时间均短于对照组,差异有统计学意义(P<0.05)。结论肺癌患者围手术期采用基于时间护理的呼吸训练联合排痰管理,不仅能够增强患者自护能力,改善患者肺功能,还能减少术后并发症,减轻术后疼痛感,加快术后康复进展。Objective To explore the effect of respiratory training combined with expectoration management based on time nursing in the perioperative period of lung cancer patients.Methods Sixty lung cancer surgical patients who admitted to the Jiujiang Third People's Tumor Hospital from January 2021 to August 2023 were selected as the study subjects.According the random number table method,they were divided into the control group(30 cases)and the study group(30 cases).The control group received routine care,while the study group received expectoration management based on time nursing.The self-management ability score,lung function indicators,incidence of postoperative complications,postoperative pain score,and postoperative recovery time were compared between two groups.Results After nursing,the scores of self-management ability in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).After nursing,the forced expiratory volume in 1 second(FEV1)and the forced expiratory volume in 1 second/forced vital capacity(FEV1/FVC)in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).The total incidence of postoperative complications and pain score in the study group were lower than those in the control group,and the first time of leaving bed and length of hospital stay in the study group after surgery were shorter than those in the control group,with statistical significances(P<0.05).Conclusion The application of respiratory training combined with expectoration management based on time nursing in the perioperative period of lung cancer patients can not only enhance patients'self-care ability,improve patients'lung function,but also reduce postoperative complications,relieve postoperative pain,and accelerate postoperative rehabilitation progress.
