摘 要:目的:探究宫颈癌患者细胞角蛋白(Cytokeratin,CK)19表达水平对病人预后预测价值分析。方法:选取2018年4月-2022年4月我院确诊宫颈癌患者102例,均行宫颈癌根治术,同时纳入50例门诊健康女性受试者作为对照组,采用ELISA法测量血清CK19水平;采用Spearman相关分析、Kaplan-Meier分析法及Cox回归法分析CK19与各临床因素关系及对患者生存情况的影响。结果:复发组及未复发组的CK19水平明显高于对照组(P<0.05),复发患者入组时CK19水平明显高于未复发患者(P<0.05)。CK19水平与患者FIGO分期、分化程度及淋巴结转移明显相关(P<0.05);CK19水平与年龄、病理分型无明显相关(P>0.05)。生存分析显示,年龄(P=0.018)、CK19水平(P=0.004)、FIGO分期(P=0.004)及淋巴结转移(P=0.038)是宫颈癌的预后影响因素(P<0.05)。多因素分析显示,CK19水平[OR1.097,95CI%(1.053,1.144),P<0.05]及淋巴结转移是独立危险因素[OR1.411,95CI%(1.124,1.771),P<0.05]。结论:CK19对宫颈癌病人的预后判断有积极的作用,可作为辅助预后判断因子。Objective:To explore the value of cytokeratin(CK)19 expression in predicting the prognosis of patients with cervical cancer.Methods:102 patients with cervical cancer diagnosed in our hospital from April 2018 to April 2022 were treated with radical operation of cervical cancer.Serum CK19 level was measured by ELISA.The relationship between CK19 and clinical factors and its influence on survival were analyzed by Spearman correlation analysis,Kaplan-Meier analysis and Cox regression.Results:CK19 levels in relapse group and non-relapse group were significantly higher than those in normal group(P<0.05),and CK19 levels in relapse group were significantly higher than those in non-relapse group(P<0.05).CK19 level was significantly correlated with FIGO stage,differentiation degree and lymph node metastasis(P<0.05),but not with age and pathological type(P>0.05).Survival analysis showed that age(P-0.018),CK19 level(P=0.004),FIGO stage(P=0.004)and lymph node metastasis(P=0.038)were the prognostic factors of cervical cancer(P<0.05).Multivariate analysis showed that CK19 level[OR 1.097,95C1%(1.053,1.144),P<0.05]and lymph node metastasis were independent risk factors[OR 1.411,95CI%(1.124,1.771),P<0.05].Conclusion:CK19 has a positive effect on the prognosis of cervical cancer patients and can be used as an adjuvant prognostic factor.
