化瘀通络针刺法对原发性三叉神经痛(气滞血瘀证)疗效观察

摘　　要：目的观察化瘀通络针刺法对原发性三叉神经痛(PTN)(气滞血瘀证)患者的疗效。方法68例患者随机分为两组各34例。对照组采取卡马西平片治疗;治疗组于对照组的基础上采取化瘀通络针刺法治疗。两组均观察2周。比较两组的治疗效果、疼痛指标视觉模拟量表(VAS)与气滞血瘀证评分、血清神经递质与炎症因子水平。结果连续用药2周后,治疗组总有效率为91.18%,高于对照组的67.65%(P<0.05)。治疗1周与2周后,两组VAS评分显著减少,且治疗组减少更明显(P<0.05)。治疗1周与2周后,两组气滞血瘀证评分显著减少,且治疗组减少更明显(P<0.05)。连续用药2周后,两组血清P物质水平明显减少、β-内啡肽水平显著增加,同时治疗组上述指标改善更明显(P<0.05)。连续用药2周后,两组血清白细胞介素-6、肿瘤坏死因子-α水平显著下降,治疗组下降更明显(P<0.05)。结论化瘀通络针刺法对PTN(气滞血瘀证)的疗效明显,能减轻患者疼痛,改善中医证候,可调节疼痛相关神经递质水平,抑制炎症反应。Objective:To investigate the efficacy of Huayu Tongluo acupuncture on primary trigeminal neuralgia(PTN)patients with syndrome of qi stagnation and blood stasis.Methods:A total of 68 PTN cases and were randomly divided into two groups with 34 cases in each group.The control group was treated with carbamazepine tablets,and the treatment group received Huayu Tongluo acupuncture on the basis of the control group.Both groups were observed for 2 weeks.The therapeutic effect,visual analogue scale(VAS)pain scores,score of the syndrome of qi stagnation and blood stasis,serum levels of neurotransmitter and inflammatory factors were compared between the two groups.Results:After 2 weeks'continuous treatment,the total effective rate in the treatment group(91.18%)was evidently higher than the control group(67.65%)(P<0.05).After 1 week and 2 weeks'treatment,the VAS scores for both groups were evidently decreased,and the treatment group decreased more evidently(P<0.05).After 1 week and 2 weeks'treatment,scores of qi stagnation and blood stasis syndrome for both groups were evidently reduced,and treatment group decreased more evidently(P<0.05).After 2 weeks'continuous treatment,serum level of substance P was evidently decreased and the level ofβ-endorphin was evidently increased for both groups,and the above indexes for treatment group improved more evidently(P<0.05).After 2 weeks of continuous treatment,serum levels of interleukin-6 and tumor necrosis factor-αfor both groups were evidently decreased,and the reduction for treatment group was more evident(P<0.05).Conclusion:Huayu Tongluo acupuncture has an obvious curative effect on PTN patients with syndrome of qi stagnation and blood stasis,which can reduce pain,improve traditional Chinese medicine syndromes,regulate the level of pain-related neurotransmitters,and inhibit inflammatory response.
