生物免疫联合化疗对转移性前列腺癌患者疗效、T淋巴细胞计数及生活质量的影响

摘　　要：目的研究生物免疫联合化疗对转移性前列腺癌患者疗效、T淋巴细胞计数及生活质量的影响。方法选取陕西核工业215医院在2020年1月至2023年1月收治的80例转移性前列腺癌患者纳入研究,按随机数字表法分为观察组和对照组,每组各40例。记录两组治疗效果,比较两组免疫组织反映、T淋巴亚群及生活质量调查表(quality of life questionnaire PORTEC-30,QLQ-C30)水平,观察两组不良反应。结果观察组治疗效果显著优于对照组(Z=4.98,P<0.05);治疗后观察组T辅助细胞、T抑制细胞、自然杀伤性T细胞、自然杀伤细胞、总T淋巴细胞百分比高于对照组(t值分别为3.96、4.74、9.81、7.99、4.62,P值均<0.05);治疗后观察组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)高于对照组,CD8^(+)低于对照组(t值分别为4.65、5.87、4.71、3.80,P值均<0.05);治疗后观察组角色、躯体、情绪、认知、社会功能评分高于对照组(t值分别为7.85、4.05、6.09、9.48、6.48,P值均<0.05);观察组不良反应发生率低于对照组(χ^(2)=9.60,P<0.05)。结论生物免疫联合化疗治疗转移性前列腺癌患者疗效显著,能够有效改善免疫组织反映及T淋巴亚群水平,提高患者生活质量,且能够降低不良反应,值得临床推广应用。ObjectiveStudy the effects of biological immunotherapy combined with chemotherapy on the efficacy,T lymphocyte count,and quality of life of patients with metastatic prostate cancer.MethodsA total of 80 patients with metastatic prostate cancer admitted to the 215 Hospital of Shaanxi Nuclear Industry from January 2020 to January 2023 were included in the study,and were randomly divided into observation group and control group,40 cases in each group.Record the therapeutic effects of two groups,compare the levels of immune tissue response,T lymphocyte subsets,and quality of life questionnaire(Quality of life questionnaire PORTEC-30,QLQ-C30)between the two groups,and observe the adverse reactions of the two groups.ResultsThe treatment effect of the observation group was significantly better than that of the control group(Z=4.98,P<0.05);After treatment,the percentage of T helper cells,T suppressor cells,natural killer T cells,natural killer cells,and total T lymphocytes in the observation group was higher than that in the control group(t values were 3.96,4.74,9.81,7.99,4.62,all P values<0.05);After treatment,the CD3^(+),CD4^(+),CD4^(+)/CD8^(+)levels in the observation group were higher than those in the control group,while CD8^(+)levels were lower than those in the control group(t values were 4.65,5.87,4.71,3.80,all P values<0.05);After treatment,the role,body,emotion,cognition,and social function scores of the observation group were higher than those of the control group(t values were 7.85,4.05,6.09,9.48,6.48,all P values<0.05);The incidence of adverse reactions in the observation group was lower than that in the control group(χ^(2)=9.60,P<0.05).ConclusionThe combination of biological immunity and chemotherapy has a significant therapeutic effect on patients with metastatic prostate cancer,which can effectively improve immune tissue response and T lymphocyte subgroup levels,improve patient quality of life,and reduce adverse reactions.It is worthy of clinical promotion and application.
