CK5/6、TTF-1 双染联合弹力纤维染色在肺癌胸膜侵犯诊断中的应用

摘　　要：目的探讨TTF⁃1、CK5/6双染联合弹力纤维染色在肺癌脏层胸膜侵犯诊断中的应用价值。方法对108例肺癌手术标本分别进行TTF⁃1、CK5/6双染联合弹力纤维染色和传统弹力纤维染色,对比两种染色方法在判断胸膜侵犯中的差异。结果TTF⁃1和CK5/6联合弹力纤维染色检出胸膜侵犯者44例,在肺腺癌中癌细胞核呈棕黄阳性(TTF⁃1+),肺鳞癌中癌细胞质被染成鲜红色(CK5/6+),弹力纤维呈深紫色,可以清晰显示肿瘤细胞与弹力纤维的关系;而传统弹力纤维染色中癌细胞显示不清,检出胸膜侵犯者仅40例。结论TTF⁃1、CK5/6双染联合弹力纤维染色的“三重染色”方法,使得胸膜侵犯的判断变得更加直观、简单,并有助于提高胸膜侵犯的检出率。Objective To investigate the utility of CK5/6 and TTF-1 dual immunostaining combined with elastin fiber staining in diagnosing visceral pleural invasion in lung cancer.Methods A total of 108 lung cancer surgical specimens were subject to TTF-1 and CK5/6 dual immunostaining combined with elastic fiber staining,as well as traditional elastic fiber staining.A comparison was made between the two staining methods in determining pleural invasion.Results The combined TTF-1 and CK5/6 dual immunostaining with elastic fiber staining detected pleural invasion in 44 cases.In lung adenocarcinoma,cancer cell nuclei were stained brown-yellow(TTF-1+),while in lung squamous cell carcinoma,cancer cell cytoplasm was stained bright red(CK5/6+),and elastic fibers appeared deep purple,clearly demonstrating the relationship between tumor cells and elastic fibers.In contrast,traditional elastic fiber staining showed unclear visualization of cancer cells,detecting pleural invasion in only 40 cases.Conclusion The"triple staining"method of TTF-1 and CK5/6 dual immunostaining combined with elastic fiber staining provides a more intuitive and straightforward approach toassessing pleural invasion, thereby enhancing the detection rate of pleural invasion.
