摘 要:目的应用口腔锥体束CT(CBCT)图像评估上颌磨牙根尖与上颌窦底的距离及位置关系,为临床决策提供参考。方法回顾性分析口腔颌面外科112例患者的颌面部CBCT图像。根据体质量指数(body mass index,BMI)、性别、年龄分组,从近远中向测量根尖与上颌窦底的距离,从颊舌向观察上颌磨牙根尖与上颌窦底垂直关系。结果年龄组:青年组各个磨牙的三根垂直距离均低于中年组,P<0.05。性别组:男性和女性各个磨牙的三根垂直距离组间比较,差异均无统计学意义。BMI组:偏胖组在左上第二恒磨牙的近中颊根和腭根垂直距离大于正常组,P<0.05,其余不同BMI分层牙根垂直距离的比较,差异均无统计学意义。结论年龄越小,磨牙根尖与上颌窦底之间的距离越小。BMI对磨牙根尖与上颌窦底之间的距离影响较小。性别差异不影响磨牙根尖与上颌窦底之间的距离。Objective CBCT images were used to evaluate the distance and position relationship between maxillary molar apex and maxillary sinus base,so as to provide reference for clinical decision-making.Methods The maxillofacial CBCT images of 112 patients were retrospectively analyzed in the Oral and Maxillofacial Surgery Department.According to body mass index(BMI),gender and age groups,the distance between the root apex and the maxillary sinus base was measured from the proximal and distal direction,and the vertical relationship between the root and the maxillary sinus base was observed from the buccal and lingual direction.Results The vertical distance of three molars in the young group was lower than that in the middle-aged group,with P<0.05 in age groups.There was no significant difference in the vertical distance between male and female molars in gender groups.The vertical distance between the mesiobuccal root and the palatal root of the left upper second permanent molar in the overweight group was greater than that in the normal group in BMI groups,with a statistically significant difference(P<0.05).The vertical distance of the root of other different BMI layers was not statistically significant.Conclusion The younger the age,the smaller the distance between the molar root and maxillary sinus.BMI had little effect on the distance between molar root and maxillary sinus base.The gender difference did not affect the distance between molar root and maxillary sinus base.
