摘 要:目的 通过观察胶东地区成年人口腔锥形束CT(cone beam computed lomography, CBCT)结果,对下颌神经管的分支在下颌骨内走行进行分析统计,为口腔医学临床工作尤其是牙槽外科、口腔种植科操作提供准确的临床影像数据。方法 选取2021年12月到2022年1月期间在烟台市口腔医院拍摄CBCT影像且符合纳入标准的188例成年人(男性78人,女性110人,年龄18~59岁)患者的口腔CBCT数据,应用NNT图像系统分析观察,汇总统计分支发生率。结果 下颌神经管分支发生率为52.12%,侧别发生率为43.35%,其中Ⅰ、Ⅱ、Ⅲ、Ⅳ、Ⅴ型的发生率分别为11.70%、9.30%、9.84%、6.38%、6.11%。男性和女性左侧和右侧下颌神经管分支的发生率的差异无统计学意义(P>0.05)。结论 CBCT相较于平片更能够清晰显示下颌神经管及其分支的解剖特征,对于牙槽外科及口腔种植科进行手术时,可以更细致进行术前设计,避免损伤神经分支。Objective To observe the results of cone beam computed tomography(CBCT)scanning of the oral cavity of adults in Jiaodong,analyze and count the branches of the mandibular canal in the mandible,so as to provide accurate clinical image data for the clinical work of stomatology,especially for the operations of alveolar surgery and dental implant department.Methods The oral CBCT data of 188 adult patients(78 males,110 females,aged 18-59 years old)who underwent CBCT imaging at Yantai Stomatological Hospital from December 2021 to January 2022 and met the inclusion criteria were selected.The NNT imaging system was used for analysis and observation,and the incidence of branching was summarized and statistically analyzed.Results The incidence of branches of the mandibular canal was 52.12%,and the incidence of side classification was 43.35%.The incidence of types I,II,III,IV,and V was 11.70%,9.30%,9.84%,6.38%,and 6.11%,respectively.There was no significant difference in the incidence of the branches of the left and right mandibular canals between men and women(P>0.05).Conclusion Compared with plain film,CBCT can clearly show the anatomical characteristics of the mandibular canal and its branches.When performing surgery in the alveolar surgery and dental implant department,it can be designed more carefully before surgery to avoid damaging nerve branches.
