摘　　要：目的 探讨合并焦虑抑郁对原发性青光眼患者小梁切除术后眼压及心理应激的影响。方法 选择2021年6月~2023年6月我院收治的76例原发性青光眼患者为研究对象,均进行小梁切除术。手术结束后将患者分为不良情绪组(存在焦虑抑郁情绪,共33例)与对照组(无焦虑抑郁情绪,共43例)。比较两组一般资料,包括性别、年龄、学历;手术前、术后1天、术后1周及术后1个月,检测比较两组眼压;手术前及术后3天,检测比较两组心理应激指标[去甲肾上腺素(norepinenlphrine,NE)、皮质醇(cortisol,Cor)、促肾上腺皮质激素(adrenocorticotrophic hormone,ACTH)];记录两组术后并发症发生情况。结果 两组性别、年龄、学历组间比较差异无统计学意义(P>0.05)。术后1天、术后1周,不良情绪组眼压水平显著高于对照组(P<0.05),术后1个月两组眼压比较无显著差异(P>0.05)。术后3天,不良情绪组NE、Cor、ACTH水平显著高于对照组(P<0.05)。不良情绪组并发症发生率显著高于对照组(P<0.05)。结论 合并焦虑抑郁会导致原发性青光眼患者术后眼压恢复减慢,心理应激水平升高,且术后并发症发生率更高。Objective To investigate the effects of anxiety and depression on intraocular pressure and psychological stress in patients with primary glaucoma after trabeculectomy.Methods A total of 76 patients with primary glaucoma admitted to our hospital from June 2021 to June 2023 were selected as the study objects,all of whom underwent trabeculectomy.After the operation,the patients were divided into negative emotion group(33 cases with anxiety and depression)and control group(43 cases without anxiety and depression).The general data of the two groups were compared,including gender,age,education background.Before operation,1 day after operation,1 w after operation and 1 month after operation,the intraocular pressure of the two groups was detected and compared.The psychological stress indexes [norepinephrine(NE),cortisol(Cor),adrenocorticotrophic hormone(ACTH)] of the two groups were detected and compared before and 3 days after operation.The postoperative complications of the two groups were recorded.Results There was no significant difference in gender,age and education between the two groups(P>0.05).At 1 d and 1 w after surgery,the intraocular pressure level of the negative emotion group was significantly higher than that of the control group(P<0.05),but there was no significant difference between the two groups one month after surgery(P>0.05).At 3 days after operation,the levels of NE,Cor and ACTH in the negative emotion group were significantly higher than those in the control group(P<0.05).The incidence of complications in the negative emotion group was significantly higher than that in the control group(P<0.05).Conclusion The combination of anxiety and depression can lead to slower recovery of intraocular pressure,higher psychological stress level and higher incidence of postoperative complications in patients with primary glaucoma after trabeculectomy.
