高眼压状态下原发性急性闭角型青光眼手术治疗的效果研究

摘　　要：目的探讨对高眼压状态下原发性急性闭角型青光眼患者实施手术治疗的临床效果。方法回顾性选取2021年8月—2023年9月莒县中医医院收治的83例高眼压状态下原发性急性闭角型青光眼患者的临床资料。依据手术方法分组,参照组(41例)选择小梁切除术完成治疗,研究组(42例)选择激光虹膜切除术完成治疗。比较两组的手术疗效、裸眼视力、并发症以及眼压控制情况。结果研究组治疗总有效率为97.62%(41/42),高于参照组的80.49%(33/41),差异有统计学意义(χ^(2)=6.298,P<0.05)。术前,两组的裸眼视力以及眼压控制情况对比,差异无统计学意义(P均>0.05),术后研究组各时间段的裸眼视力以及眼压控制情况均优于参照组,差异有统计学意义(P均<0.05)。研究组并发症总发生率低于参照组,差异有统计学意义(P<0.05)。结论对高眼压状态下原发性急性闭角型青光眼患者实施手术治疗后,患者的裸眼视力以及眼压水平得到改善,还降低了并发症发生率。Objective To investigate the clinical effect of surgical treatment of primary acute angle-closure glaucoma with high intraocular pressure.Methods The clinical data of eighty-three patients with primary acute angle-closure glaucoma under high intraocular pressure in Juxian Hospital of Traditional Chinese Medicine from August 2021 to September 2023 were retrospectively selected.The patients in the reference group(forty-one cases)were treated with trabeculectomy,while those in the study group(forty-two cases)were treated with laser iridectomy.The operative ef-fect,naked eye visual acuity,complications and intraocular pressure control were compared between the two groups.Results The total effective rate of the study group was 97.62%(41/42),which was higher than 80.49%(33/41)of the control group,the difference was statistically significant(χ^(2)=6.298,P<0.05).Before operation,there was no statisti-cally significant difference in naked eye visual acuity and intraocular pressure control between the two groups(both P>0.05).After operation,naked eye visual acuity and intraocular pressure control in all time periods of the study group were better than that of the reference group,the differences were statistically significant(all P<0.05).The total inci-dence of complications in the study group was lower than that in the reference group,and the difference was statisti-cally significant(P<0.05).Conclusion After surgical treatment of patients with primary acute angle-closure glaucoma in the condition of high intraocular pressure,the naked eye visual acuity and intraocular pressure levels were im-proved,and the incidence of complications was reduced.
