低精液量性不育的病因及治疗研究进展

摘　　要：精子进入女性生殖道内完成自然受孕需要有足够的射精量,任何导致精液量减少/低精液量的因素都会对男性生殖及健康产生不利影响。然而,临床中如果存在其他精液参数异常时,精液量的作用常被忽略。导致低精液量的原因包括人为因素、环境与心理因素及病理因素,病理因素主要有男性生殖道发育和功能异常,包括先天性双侧输精管缺如/单侧输精管缺如(CBAVD/CUAVD)、射精管梗阻(EDO)、性腺功能低下等;射精功能障碍包括逆行射精(RE)、不射精(AE)等。目前,针对低精液量性不育的病因及临床特点尚缺乏系统性专述,治疗方案仍需充分探讨。本文就近年来临床工作中常见的低精液量性不育的原因、特点及诊治进展作一综述,为临床诊治提供参考。The sperm entering into the female reproductive tract requires sufficient ejaculation to achieve natural conception.Any factor that leads to reduced semen volume/low semen volume can negatively affect male reproduction and health.However,the role of semen volume is often ignored in the clinic when other semen parameters are abnormal.The causes of low semen volume include human factors,environmental and psychological factors,and various pathological factors.The main pathological factors are abnormal development and dysfunction of male reproductive tract,including congenital bilateral absence of vas deferens/congenital unilateral absence of vas deferens(CBAVD/CUAVD),ejaculatory duct obstruction(EDO),hypogonadism,etc.Ejaculatory dysfunction includes retrograde ejaculation(RE),anejaculation(AE),etc.At present,the etiology and clinical characteristics of low semen volume type infertility are lack of systematic review,and the treatment plan still needs to be fully discussed.In this paper,the causes,characteristics,diagnosis and treatment progress of common low semen volume type infertility in clinical work in recent years are reviewed,so as to provide reference for clinical diagnosis and treatment.
