阻生智齿拔除对下颌第二磨牙牙周健康影响的临床分析

摘　　要：目的 探究阻生智齿拔除后,采用不同方法处理拔牙创口,对下颌第二磨牙牙周健康的影响.方法 选取2022年1月-2024年4月就诊于南京医科大学第四附属医院口腔科的阻生智齿拔除患者60例作为研究对象,根据对拔牙创口的处理方式不同分为观察组(n=30)和对照组(n=30),对照组采用常规明胶海绵填塞创口,观察组采用富血小板纤维蛋白填塞创口,随访记录术后1个月、3个月和6个月复诊情况,对比术前、术后下颌第二磨牙远中牙槽嵴附着高度变化和牙周袋深度,术后1周牙齿松动情况,判断阻生智齿拔除术后对下颌第二磨牙牙周健康影响.结果 术后1周,观察组的无牙齿松动率为96.67%,对照组为83.33%,观察组优于对照组,差异较为显著(P<0.05);术前和术后1个月、3个月、6个月检查下颌第二磨牙牙槽嵴高度均有轻微变化,且观察组优于对照组,但差异不显著(P>0.05);术前和术后1个月检查下颌第二磨牙远中牙周袋深度差异不显著(P>0.05),术后3个月、6个月观察组的牙周深度低于对照组,差异较为显著(P<0.05).结论 阻生智齿拔除后,采用富血小板纤维蛋白凝胶填塞手术创面,对下颌第二磨牙牙周修复具有促进作用,能够保证牙周健康.Objective To explore the effects of the extraction of impacted wisdom teeth.Methods 60 patients treated in the Department of Stomatology of the Fourth Affiliated Hospital of Nanjing Medical University in January 2022 to April 2024 were selected as the study subjects,according to the observation group(n=30)and control group(n=30),in the control group,the observation group used a platelet-rich fibrin tamponade wound,follow-up visits at 1,3 and 6 months after surgery,compared are the changes in the distal alveolar ridge attachment height and periodontal pocket depth of the second molar before and after surgery,tooth loosening at 1 week after surgery,to determine the effects of impacted wisdom tooth extraction on periodontal health of mandibular second molar.Results At 1 week after surgery,the rate of edentless loosening in the observation group was 96.67%,in the control group,83.33%,the observation group was better than the control group,the difference was more significant(P<0.05);minor changes in the ridge height of the mandibular second molar alveolar ridge were examined before and at 1,3 and 6 months after surgery,and the observation group was better than the control group,but the difference was not significant(P>0.05);examination of mandibular second molar before and 1 month after surgery,no significant difference in peripouch depth(P>0.05),lower periodontal depth in the observation group than the control group at 3,6 months after surgery,the difference was more significant(P<0.05).Conclusion After the removal of the impacted wisdom teeth,the platelet-rich fibrin gel filling surgery was used to promote the periodontal repair of the mandibular second molar and ensure the periodontal health.
