干扰素联合解毒祛湿汤外洗对高危型人乳头瘤病毒感染患者自然转归、阴道局部免疫状态的影响

摘　　要：目的:探讨干扰素(INF)联合解毒祛湿汤外洗对高危型人乳头瘤病毒(HR-HPV)感染女性患者自然转归和阴道局部免疫状态的影响。方法:选取100例HR-HPV感染女性患者为研究对象,根据治疗方式不同分为对照组和研究组,每组各50例。对照组患者给予常规INF治疗;研究组患者给予INF联合解毒祛湿汤外洗治疗,疗程均为3个月。比较两组患者临床疗效、中医证候评分、HR-HPV转阴率、病毒载量水平、炎性因子[白细胞介素12(IL-12)、白细胞介素4(IL-4)、γ干扰素(IFN-γ)]水平、阴道灌洗液人防御素5(HD-5)、人防御素2(HBD-2)水平及不良反应发生情况。方法:研究组患者临床有效率为高于对照组的(92.00%vs.76.00%,P<0.05)。治疗3个月后,研究组患者带下量多、体倦乏力、带下质稀积分低于对照组(P<0.05);HR-HPV转阴率高于对照组(82.00%vs.56.00%,P<0.05);病毒载量水平低于对照组(P<0.05);两组患者IL-12、IFN-γ水平均升高(P<0.05),且研究组高于对照组(P<0.05);IL-4、HD-5、HBD-2水平均降低(P<0.05),且研究组低于对照组(P<0.05)。两组患者不良反应发生率比较,差异无统计学意义(P>0.05)。结论:INF联合解毒祛湿汤外洗治疗HR-HPV感染患者效果好,能促进病毒的清除,改善患者的自然转归,并有助于恢复患者阴道局部免疫状态,减轻炎症反应。Objective:To explore the effects of interferon and Jiedu Qushi Decoction on natural outcomes and vaginal microecology balance in patients with high-risk human papillomavirus(HR-HPV)infection.Methods:The clinical data of 100 patients with HR-HPV infection were collected.All patients were divided into control group(n=50,routine interferon)and study group(n=50,Jiedu Qushi Decoction+routine interferon)according to different treatment methods.All patients were treated for 3 months.The clinical curative effect,scores of TCM syndromes,negative conversion rate of HR-HPV,viral load,levels of inflammatory factors[interleukin-12(IL-12),interleukin-4(IL-4),interferon-γ(IFN-γ)],human defensin-5(HD-5)and human defensin-2(HBD-2)in vaginal lavage fluid,and incidence of adverse reactions were compared between the two groups.Results:After treatment,clinical response rate was higher in study group than control group(92.00%vs.76.00%,P<0.05).After treatment,the leukorrhagia,fatigue,and low-concentration leucorrhea were lower in study group(P<0.05).The difference in negative conversion rate of HR-HPV between study group and control group was statistically significant(82.00%vs.56.00%),and viral load was lower in study group(P<0.05).After treatment,levels of IL-12 and IFN-γwere increased,while IL-4,HD-5,and HBD-2 levels were decreased in both groups.The levels of IL-12 and IFN-γwere higher in study group than control group,while IL-4,HD-5,and HBD-2 levels were lower in study group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Curative effect of interferon combined with Jiedu Qushi Decoction is significant in patients with HR-HPV infection,which can promote viral elimination,improve natural outcomes and is beneficial to maintain the balance of vaginal microecology and relieve inflammatory response.
