摘　　要：目的介绍肾嗜酸细胞瘤(RO)的临床特点,探讨该病的临床病理特点、免疫组化表型、诊断、鉴别诊断及预后。方法回顾性分析7例RO的临床资料、组织学特点及免疫组化结果并结合文献对其进行探讨。结果镜下肿瘤组织界限清晰,瘤细胞排列呈实体巢状、管状,间质疏松水肿或黏液状,间质细胞稀少。免疫组化:AE1/AE3阳性(7/7)、CD117阳性(7/7)、Pax8阳性(7/7)、S100A1阳性(6/7)、Cathepsin K阳性(5/7)、P504s阳性(1/7)、CK7个别细胞阳性(2/7)、Vimentin阳性(0/7)、CD82阳性(0/7)、Claudin7阳性(0/7)、CD10阳性(0/7)、Ki-67(1%~4%)。结论RO是一种少见的肾上皮源性肿瘤,诊断需结合组织学及免疫组化综合考虑。其中,肿瘤细胞CD117、Pax8、S100A1、Cathepsin K(+),CK7、Claudin7、CD82(-)对其诊断至关重要。治疗上目前推荐保留肾单位的肿瘤切除术,预后良好。所以正确诊断具有重要的临床意义。Objective To introduce the clinical features of renal oncocytoma,and to explore the clinicopathological features,immunohistochemical phenotype,diagnosis,differential diagnosis and prognosis of renal oncocytoma.Methods The clinical data,histological features and immunohistochemical phenotype of 7 cases of renal oncocytoma were retrospectively analyzed with the review of relative literature.Results Histologically,the tumor tissue was well circumscribed.The tumor cells were arranged in solid nests and tubes.The stroma was loose,edematous and mucous with sparse cells.Immunohistochemically,AE1/AE3 was positive(7/7),CD117 positive(7/7),Pax8 positive(7/7),S100A1 positive(6/7),Cathepsin K positive(5/7),P504s positive(1/7),CK7 partially positive(2/7),Vimentin positive(0/7),CD82 positive(0/7),Claudin7 positive(0/7),CD10 positive(0/7),and Ki-67 index(1%-4%).Conclusion RO is a rare benign renal epithelial tumor.The diagnosis needs to be considered in combination with histology and immunohistochemistry.The expression of CD117,Pax8,S100A1,Cathepsin K,CK7,Claudin7 and CD82 in tumor cells is of vital importance to its diagnosis.The nephron-sparing surgery is recommended.They have a relatively good prognosis.Therefore,correct diagnosis is of great significance for clinical treatment.
