摘　　要：目的探讨子宫内膜样癌伴性索样结构和玻璃样变(CHEC)的临床病理学特征、诊断、鉴别诊断、治疗及预后。方法收集陆军军医大学大坪医院2023年11月诊治的1例CHEC患者的临床资料,分析病理学特征,并复习相关文献。结果女性,33岁,病理诊断为CHEC,肿瘤由经典的低级别子宫内膜样癌和约10%的性索样结构及玻璃样变伴有骨化生的区域混合构成,2023版子宫内膜癌FIGO分期ⅠB期;患者术后接受放疗,目前情况良好,未见复发。结论CHEC是子宫内膜样癌的一种罕见形态变异,发病年龄较年轻,因其双相的组织学易误诊为癌肉瘤等,准确诊断依赖于病理医生对该病变的认识。一般行全子宫、双侧附件切除及区域淋巴结清扫,辅以术后放疗及化疗,大部分患者FIGO分期较早,预后较好。Objective To investigate the clinicopathological characteristics,diagnosis,differential diagnosis,treatment,and prognosis of corded and hyalinized endometrioid carcinomas(CHEC).Methods The clinical data and pathological features of one CHEC patient were analyzed with review of relevant literatures.Results A 33 year old female with pathological diagnosis of CHEC was diagnosed.The tumor was composed of a mixture of classic low-grade endometrioid carcinoma and about 10%of sex cord-like structures and hyalinization accompanied by ossification.The FIGO staging of endometrial cancer in the 2023 version was stage IB;The patient was currently undergoing radiotherapy one month after surgery and was in good condition.Conclusion CHEC is a rare morphological variation of endometrioid carcinoma,with a younger onset age.Due to its biphasic histology,it is prone to misdiagnosis as carcinosarcoma,and accurate diagnosis depends on the pathologist's understanding of the lesion.Generally,total hysterectomy,bilateral adnexectomy,and regional lymph node dissection are performed,supplemented by postoperative radiotherapy and chemotherapy.Most patients have earlier FIGO staging and a better prognosis.
