培土扶木法中药联合撮谷道治疗功能性肛门直肠痛的临床疗效观察

摘　　要：目的:观察培土扶木法中药联合撮谷道功法对功能性肛门直肠痛的影响。方法:将60例功能性肛门直肠痛患者随机分为对照组和观察组,每组30例。对照组采用四黄祛毒汤熏洗治疗,观察组采用四黄祛毒汤熏洗加培土扶木法中药口服联合撮谷道功法治疗。治疗7 d后,比较两组患者临床疗效,以及治疗前后疼痛视觉模拟评分量表(VAS),肛门坠胀评分,灼热、异物感评分,及SF-36生活质量评分。结果:治疗后,观察组治疗总有效率为86.67%(26/30),显著高于对照组的63.33%(19/30)(P<0.05);观察组治疗后VAS评分、肛门坠胀评分和灼热、异物感评分改善均显著低于同期对照组(P<0.05);SF-36调查显示,两组生理角色职能(RP)、总体健康(GH)、社会功能(SF)评分较本组治疗前显著升高(P<0.05),且观察组治疗后GH和SF评分显著高于同期对照组(P<0.05)。结论:培土扶木法中药联合撮谷道功法治疗功能性肛门直肠痛可明显缓解患者临床症状,显著提高临床疗效,突显中医药特色,值得推广。Objective:To observe the effect of Peitu Fumu method and Cuogudao on functional anorectal pain.Methods:Sixty patients with functional anorectal pain were randomly divided into a control group and an observation group,30 in each group.The control group was treated with Sihuang Qudu Decoction fumigation,while the observation group was treated with Sihuang Qudu Decoction fumigation and the Peitu Fumu method and Cuogudao.After 7 days of treatment,the clinical efficacy of the two groups of patients was compared,as well as the visual analog scale(VAS)for pain before and after treatment,anal prolapse score,burning and foreign body sensation score,and SF-36 quality of life score.Results:After treatment,the total effective rate of 86.67%(26/30)in the observation group was significantly higher than that of 63.33%(19/30)in the control group(P<0.05);After treatment,the VAS score,anal prolapse score,burning sensation,and foreign body sensation score in the observation group were significantly lower than those in the control group at the same time(P<0.05);The SF-36 survey showed that role physical(RP),general health(GH),and social functioning(SF)scores of both groups were significantly higher than before treatment in this group(P<0.05),and the GH and SF scores of the observation group were significantly higher than those of the control group at the same time after treatment(P<0.05).Conclusion:The combination of the traditional Chinese medicine method of nourishing soil and supporting wood with the Cuogudao technique can significantly alleviate clinical symptoms of functional anorectal pain in patients,significantly improve clinical efficacy,highlight the characteristics of traditional Chinese medicine,and is worthy of promotion.
