腹腔镜下胃穿孔修补术与开腹胃穿孔修补术治疗胃溃疡合并胃穿孔患者的效果比较

摘　　要：目的比较腹腔镜下胃穿孔修补术(LRGP)与开腹胃穿孔修补术(ORGP)治疗胃溃疡合并胃穿孔患者的临床效果。方法将100例胃溃疡合并胃穿孔患者随机分为两组各50例。微创组行LRGP治疗,开腹组行ORGP治疗。比较两组的围术期指标、胃功能指标及术后并发症发生情况。结果微创组术后首次排气时间、肠鸣音恢复时间、下床活动时间及住院时间短于开腹组(P<0.05)。术后5 d,微创组血清胃泌素、抑胃肽、胃动素水平高于开腹组(P<0.05)。微创组术后并发症发生率为6.00%,低于开腹组的22.00%(P<0.05)。结论与ORGP治疗相比,LRGP治疗可促进胃溃疡合并胃穿孔患者术后尽快恢复,可明显改善患者的胃功能,减少术后并发症的发生。Objective To compare the clinical effects of laparoscopic repair of gastric perforation(LRGP)and open repair of gastric perforation(ORGP)in the treatment of patients with gastric ulcer complicated with gastric perforation.Methods 100 patients with gastric ulcer complicated with gastric perforation were selected and randomly divided into two groups,with 50 cases in each group.The minimally invasive group was treated with LRGP,and the open surgery group was treated with ORGP.The perioperative indicators,gastric function indicators and incidence of postoperative complications were compared between the two groups.Results The first exhaust time,bowel sound recovery time,out-of-bed activity time and hospitalization time of the minimally invasive group were shorter than those of the open surgery group(P<0.05).5 days after operation,the serum gastrin,gastric inhibitory peptide and motilin levels of the minimally invasive group were higher than those of the open surgery group(P<0.05).The incidence of postoperative complications in the minimally invasive group was 6.00%,lower than 22.00%in the open surgery group(P<0.05).Conclusions Compared with ORGP,LRGP can promote the postoperative recovery of patients with gastric ulcer complicated with gastric perforation as soon as possible,and can significantly improve the gastric function of patients,and reduce the postoperative complications.
