常规清醒脑电图与睡眠剥夺视频脑电图在癫痫诊断中的应用价值

摘　　要：目的探究癫痫患者应用常规清醒脑电图和睡眠剥夺睡眠视频脑电图(VEEG)的诊断效果。方法选取南京医科大学附属淮安第一医院2020-09—2023-09收治的98例癫痫患者,所有患者均行常规清醒脑电图和睡眠剥夺VEEG检查,比较两种检查方法癫痫阳性检出率,比较阳性、阴性患者(睡眠剥夺VEEG检查)的基本资料,采用Logistic多因素回归模型分析诊断阳性率的影响因素。结果常规清醒脑电图阳性检出率为52.04%(51/98),其中两侧中央、颞区尖波、尖慢波散发25例,两侧中央、颞区尖波、尖慢波散发26例。睡眠剥夺VEEG阳性检出率为83.67%(82/98),其中广泛高-极高波幅棘慢波暴发46例,36例睡眠期左侧颞区有尖慢波、尖波散发。与常规清醒脑电图相比,睡眠剥夺VEEG阳性检出率更高(P<0.05);阳性患者(睡眠剥夺VEEG检查)发作持续时间(42.21±2.69)长于阴性患者(38.25±2.56),阳性患者发作次数(26.23±2.39)多于阴性患者(21.05±2.36,P<0.05)。Logistic分析显示,发作持续时间和次数是影响诊断阳性率(睡眠剥夺VEEG)的独立因素(P<0.05)。结论与常规清醒脑电图相比,睡眠剥夺睡眠视频脑电图阳性检出率更高。发作持续次数、时间能够独立影响睡眠剥夺VEEG诊断阳性率。Objective To investigate the diagnostic efficacy of conventional awake EEG and sleep�deprived video EEG(VEEG)in patients with epilepsy.Methods A total of 98 patients with epilepsy admitted to the Affiliated Huaian No.1 People’s Hospital of Nanjing Medical University from September 2020 to September 2023 were selected.All patients underwent routine awake EEG and sleep-deprived VEEG tests to compare the positive rate of epilepsy diagnosis between the two methods.Basic data of positive and negative patients(as determined by sleep-deprived VEEG)were compared,and the factors influencing the positive diagnosis rate were analyzed using Logistic multivariate regression model.Results The positive detection rate of routine awake EEG was 52.04%(51/98),with 25 cases showing bilateral central and temporal sharp wave and sharp slow wave,while 26 cases showed bilateral central and temporal sharp wave and sharp slow wave.The positive detection rate of sleep-deprived VEEG was 83.67%(82/98),among which 46 cases exhibited widespread high-to-extremely high amplitude spike slow wave activity,while 36 cases showed sharp slow waves and sharp wave emissions in the left temporal region during sleep deprivation.The positive rate of VEEG during sleep deprivation was significantly higher than that during conventional awake EEG testing(P<0.05).The duration of sleep deprivation VEEG in positive patients(42.21±2.69)was longer than that in negative patients(38.25±2.56),and the number of seizures in positive patients(26.23±2.39)was higher than that in negative patients(21.05±2.36,P<0.05).Logistic analysis revealed that seizure duration and frequency were independent factors affecting the positive diagnosis rate for VEEG during sleep deprivation(P<0.05).Conclusion In comparison to conventional awake EEG,the use of sleep-deprived video EEG resulted in a higher positivity detection rate for epilepsy diagnosis.Furthermore,seizure duration and frequency independently influence the positivity rate for VEEG diagnosis during sleep deprivation.
