运动疗法联合振动训练介入时机对急性缺血性脑卒中患者下肢功能的影响

摘　　要：目的探讨急性缺血性脑卒中(AIS)患者在不同时期介入运动疗法联合振动训练对下肢功能的效果。方法选取郑州大学附属郑州中心医院康复科2022-03—2022-08诊治的84例AIS患者,均采取常规运动疗法和振动训练治疗,观察组患者发病时间≤2周,对照组患者发病时间>2~12周,2组各42例,试验中2组各有2例脱落。比较治疗前后2组患者Fugl-Meyer(FMA)下肢功能评分、Barthel指数(BI)、Holden功能性步行量表相关指标。结果治疗10 d后观察组FMA评分[(23.1±6.18)分比(26.05±6.21)分]、BI评分[(65.63±19.32)分比(71.88±19.17)分]、Holden步行功能分级(3.07±0.82比3.35±1.07)与对照组比较无统计学差异(P>0.05);治疗20 d后观察组FMA评分[(30.25±4.17)分比(28.07±5.52)分]、Holden步行功能分级(4.17±0.84比3.65±1.12)高于对照组(P<0.05),BI评分[(82.50±17.93)分比(77.51±19.54)分]与对照组比较无统计学差异(P>0.05)。结论AIS患者在发病2周内介入运动疗法联合振动训练对患者下肢功能恢复、日常生活能力和步行能力的恢复效果更好。Objective To investigate the effect of interventional exercise therapy combined with vibration training on lower limb function in patients with acute ischemic stroke(AIS)at different periods.Methods Eighty-four AIS patients diagnosed and treated in the Rehabilitation Department of Zhengzhou Central Hospital Affiliated to Zhengzhou University from March 2022 to August 2022 were selected,and all of them were treated by conventional exercise therapy and vibration training.The observation group with onset time≤2 weeks,and the control group with onset time>2-12 weeks,with 42 cases in each group,and 2 cases fell out in each group of the trail.The Fugl-Meyer assessment(FMA),Barthel index,and Holden functional ambulation category scale of the two groups before and after treatment were compared.Results After 10 days of treatment,the FMA score(23.1±6.18),Barthel index(65.63±19.32),and Holden walking function rating(3.07±0.82)of the observation group were not comparable to the control group(P>0.05).After 20 days of treatment,the FMA score(30.25±4.17)of the observation group was higher than the control group(28.07±5.52),and the Holden walking function rating(4.17±0.84)was higher than the control group(3.65±1.12),while the Barthel index(82.50±17.93)was not comparable to the control group(77.51±19.54)(P>0.05).Conclusion AIS patients underwent exercise therapy combined with vibration training within 2 weeks of onset have better rehabilitation effects.
