摘 要:目的分析不同程度宫颈病变患者的阴道菌群差异。方法收集2024年1月至3月在宁波大学附属妇女儿童医院接受宫颈癌筛查的100例育龄女性阴道拭子样本,根据病理学与人乳头状瘤病毒(HPV)检查结果分为对照组(20例,HPV阴性)、宫颈炎组(21例)、低级别鳞状上皮内病变组(20例)、高级别鳞状上皮内病变组(21例)、宫颈癌组(18例)。采用16S rDNA测序分析各组阴道菌群情况。结果随着宫颈病变加重,乳杆菌属丰度下降,加德纳菌属、纤毛菌属、普雷沃氏菌属、戴阿利斯特菌属等逐渐占据主导地位。结论乳杆菌减少与宫颈病变密切相关,可导致兼性和专性厌氧菌增加,及时补充阴道内乳杆菌属对预防宫颈癌具有重要意义。Objective To analyze the differences of vaginal flora in patients with different degrees of cervical lesions.Methods A total of 100 vaginal swab samples from women of childbearing age who underwent cervical cancer screening in Women and Children’s Hospital of Ningbo University from January to March 2024 were collected.According to the results of pathology and human papilloma virus(HPV)test,the patients were divided into control group(20 cases,HPV negative),cervicitis group(21 cases),low-grade squamous intraepithelial lesion group(20 cases),high-grade squamous intraepithelial lesion group(21 cases),and cervical cancer group(18 cases).16S rDNA sequencing was used to analyze the vaginal microbiota of each group.Results With the aggravation of cervical lesions,the abundance of Lactobacillus decreased,Gardnerella,Leptotrichia,Prevotella,and Dialisteria gradually became dominant.Conclusion The decrease of Lactobacillus is closely related to cervical lesions,which can lead to the increase of facultative and obligate anaerobes.Timely supplementation of Lactobacillus in the vagina is of gre at significance for the prevention of cervical cancer.
