微球囊压迫术与微血管减压术治疗三叉神经痛的有效性及安全性对比

摘　　要：目的比较微球囊压迫术与微血管减压术治疗三叉神经痛的有效性及安全性,为临床治疗提供参考。方法选取2023年1月至2024年1月临沂市中心医院收治的144例三叉神经痛患者的临床资料,进行回顾性分析。依据治疗方案不同分为微血管组(67例,接受微血管减压术治疗)和微球囊组(77例,接受微球囊压迫术治疗)。比较两组患者临床疗效、不良反应发生情况、复发情况、氧化应激指标。结果两组患者临床疗效、治疗总有效率、复发率比较,差异均无统计学意义(均P>0.05)。微血管组患者不良反应总发生率低于微球囊组(P<0.05)。术后,两组患者去甲肾上腺素(NE)、皮质醇(Cor)、髓过氧化物酶(MPO)水平均升高,且微血管组均高于微球囊组(均P<0.05)。结论微球囊压迫术、微血管减压术治疗三叉神经痛患者效果均较好,均可有效降低复发风险。但微球囊压迫术对患者的应激刺激更小,微血管减压术的安全性更高,临床医师可结合患者的实际情况合理选择。Objective To compare the efficacy and safety of microballoon compression and microvascular decompression in the treatment of trigeminal neuralgia,and to provide reference for clinical treatment.Methods 2023 was selected The clinical data of 144 patients with trigeminal neuralgia admitted to Linyi Central Hospital from January to January 2024 were retrospectively analyzed.According to different treatment regimens,they were divided into microvascular group(67 cases,treated with microvascular decompression)and microballoon group(77 cases,treated with microballoon compression).The clinical efficacy,occurrence of adverse actions,recurrence and oxidative stress indexes were compared between the two groups.Results There were no significant differences in clinical efficacy,total effective rate and recurrence rate between the two groups(allP>0.05).The total incidence of adverse reactions in the microvascular group was lower than that in the microballoon group(P<0.05).After surgery,the levels of norepinephrine(NE),cortisol(Cor)and myeloperoxidase(MPO)in the two groups were increased,and the levels of microvascular group were higher than those in the microballoon group(allP<0.05).Conclusion Microballoon compression and microvascular decompression have good effects in the treatment of patients with trigeminal neuralgia,and both can effectively reduce the risk of recurrence.However,microballoon compression has less stress stimulation on patients,and microvascular decompression is safer,so clinicians can make reasonable choices based on the actual situation of patients.
