左侧额叶CT值与偏执型精神分裂症患者阳性和阴性症状量表评分病程的相关性

摘　　要：目的分析左侧额叶CT值与偏执型精神分裂症患者阳性和阴性症状量表(PANSS)评分、病程的相关性。方法选定平顶山市精神病医院2022年1月至2024年1月就诊的120例偏执型精神分裂症患者设为观察组,以及同期体检中心120名健康体检者设为健康组,均给予头颅CT扫描,比较2组左侧额叶CT值,比较PANSS阳性(PANSS>1分)、阴性组(PANSS≤1分)患者左侧额叶CT值,比较不同病程(<1年、≥1年)组左侧额叶CT值,Pearson分析左侧额叶CT值与PANSS评分、病程的相关性。结果观察组左侧额叶CT值较健康组更低(P<0.05)。PANSS阳性组左侧额叶CT值与PANSS阴性组比较,差异无统计学意义(P>0.05)。病程≥1年组左侧额叶CT值与病程<1年组比较,差异无统计学意义(P>0.05)。左侧额叶CT值与PANSS评分、病程均呈负相关性(r=-0.441、-0.409,P=0.025、0.031)。结论偏执型精神分裂症患者左侧额叶CT值异常偏低,左侧额叶CT值与PANSS评分、病程均呈负相关性,可辅助临床对患者病情作出评价。Objective To analyze the correlation between left frontal lobe CT values and positive and negative symptom scale(PANSS)scores and disease duration in patients with paranoid schizophrenia.Methods One hundred and twenty patients with paranoid schizophrenia who visited Pingdingshan Psychiatric Hospital from January 2022 to January 2024 were selected as the observation group,and 120 healthy individuals who underwent physical examinations at the same time were selected as the health group.Both groups were given head CT scans.The left frontal lobe CT values of the two groups were compared,and the left frontal lobe CT values of PANSS positive(>1 score)and negative groups(≤1 score)were compared.The left frontal lobe CT values of different disease course(<1 year,≥1 year)groups were compared.Pearson analysis was conducted to examine the correlation between left frontal lobe CT values and PANSS scores and disease course.Results The CT value of the left frontal lobe in the observation group was lower than that in the healthy group(P<0.05).There was no significant difference in CT values between the left frontal lobe of the ANSS positive group and the PANSS negative group(P>0.05).There was no significant difference in left frontal lobe CT values between the group with a disease course of≥1 year and the group with a disease course of<1 year(P>0.05).The CT value of the left frontal lobe is negatively correlated with PANSS score and disease course(r=-0.441、-0.409,P=0.025、0.031).Conclusion Patients with paranoid schizophrenia have abnormally low left frontal lobe CT values,which are negatively correlated with PANSS scores and disease duration,and can assist clinical evaluation of the patient′s condition.
