角膜溃疡清创术联合抗真菌药物治疗急诊真菌性角膜炎的效果观察

摘　　要：目的分析对于真菌性角膜炎,采取抗真菌药物联合角膜溃疡清创术治疗的应用价值。方法选择2021年2月至2024年2月在银川进川眼科医院进行诊治的86例急诊真菌性角膜炎患者,根据抽签法随机分为试验组(抗真菌药+角膜溃疡清创术,n=43)和对照组(抗真菌药,n=43)。比较两组研究对象在症状消退时间、临床疗效、视力恢复情况上的差异。结果试验组眼部异物感消退所需时间、畏光流泪消退所需时间、积脓消退所需时间、溃疡消退所需时间均显著短于对照组研究对象(P<0.001)。试验组中41例患者临床总有效(95.35%),显著高于对照组的35例(81.40%),差异有统计学意义(P<0.05)。手术后试验组研究对象视力大部分在0.5及以上(占55.81%),其次为0.3~0.5(占30.23%);观察组研究对象视力大部分在0.05~0.3(占34.88%);试验组患者视力恢复情况明显优于对照组(P=0.001)。结论对于真菌性角膜炎,采取抗真菌药物联合角膜溃疡清创术可有效缩短症状消退时间,提高临床疗效,促进患者视力的恢复。Objective To analyze the application value of antifungal medications combined with corneal ulcer debridement in the treatment of fungal keratitis.Methods A total of 86 patients with acute fungal keratitis treated at Yinchuan Jinchuan Eye Hospital from February 2021 to February 2024 were selected.Patients were randomly assigned to either the experimental group(antifungal medication+corneal ulcer debridement,n=43)or the control group(antifungal medication only,n=43)using a lottery method.Differences between the two groups were compared in terms of symptom resolution time,clinical efficacy,and visual recovery.Results The time required for the resolution of foreign body sensation,photophobia,tearing,purulent discharge,and ulceration in the experimental group was significantly shorter than that in the control group(P<0.001).In the experimental group,41 patients achieved a total clinical effective rate of 95.35%,significantly higher than the 35 patients in the control group with a clinical effective rate of 81.40%(P<0.05).Postoperatively,the majority of patients in the experimental group had a visual acuity of 0.5 or above(55.81%),followed by 0.3 to 0.5(30.23%).In contrast,most patients in the control group had a visual acuity between 0.05 and 0.3(34.88%).The visual recovery in the experimental group was significantly better than that in the control group(P=0.001).Conclusion The combination of antifungal medications and corneal ulcer debridement is effective in reducing symptom resolution time,improving clinical efficacy,and promoting visual recovery in patients with fungal keratit is.
