摘　　要：目的:分析i-Scan联合放大内镜在筛查右半结肠息肉样病变中的应用效果。方法:选取2023年5月—2024年3月在黔东南州人民医院消化内科门诊或住院的首次进行肠镜检查的患者200例,采用随机数字表法分为i-Scan+放大内镜组与普通白光内镜组,每组100例。比较两组2次退镜后右半结肠息肉及腺瘤的检出情况,并分析第2次退镜时多检出息肉及腺瘤的特点。结果:最终入组187例,其中i-Scan+放大内镜组95例,普通白光内镜92例。第1次退镜检查两组息肉检出情况比较,差异无统计学意义(P> 0.05)。第2次退镜i-Scan+放大内镜组患者息肉及腺瘤数量、检出率、多检出息肉及腺瘤的患者比例及发现的直径<5 mm的息肉所占比例均高于普通白光内镜组,差异有统计学意义(P <0.05)。结论:i-Scan联合放大内镜模式退镜可以增加右半结肠息肉样病变的检出率,能够提高多发息肉及腺瘤的检出,与白光模式相比,能检出更多的直径<5 mm的小息肉。Objective To analyze the application effect of i-Scan combined with magnifying endoscope in the detection of polypoid lesions in the right colon.Methods A total of 200 patients who underwent colonoscopy for the first time in the Department of Gastroenterology,Qiandongnan People's Hospital from May 2023 to March 2024 were selected.They were randomly divided into i-Scan+magnifying endoscope group and common white light endoscopy group,with 100 cases in each group.The detection of polyps and adenomas in the right colon was compared between the two groups after two times of endoscopic withdrawal,and the characteristics of polyps and adenomas detected in the second time were analyzed.Results A total of 187 patients were enrolled,including 95 in the i-Scan+magnifying endoscope group and 92 in the white light endoscopy group.There was no significant difference in the detection of polyps between the two groups in the first time of endoscopic withdrawal(P>0.05).The number of polyps and adenomas,thedetection rate of patients,the rate of patients with multiple polyps and adenomas,and the proportion of polyps with a diameter of<5 mm in the i-Scan+magnifying endoscope group were significantly higher than those in the white light endoscopy group in the second time of endoscopic withdrawal(P<0.05).Conclusions i-Scan combined with magnifying endoscopy can increase the detection rate of polypoid lesions in the right colon,and improve the detection of multiple polyps and adenomas.Compared with white light mode,more polyps smaller than 5 mm can be detected.
