摘 要:目的:探究主动循环呼吸训练联合渐进式肌肉放松训练在肺癌康复期的应用效果。方法:选择2022年1月至2023年12月黄河三门峡医院收治的80例肺癌康复期患者,抽签法随机分为两组,各40例。对照组采用渐进式肌肉放松训练,观察组采用主动循环呼吸训练联合渐进式肌肉放松训练。比较两组患者的肺功能、血清炎症因子、呼吸困难程度、癌因性疲乏和并发症情况。结果:干预后,观察组患者最大呼气压(PEmax)、呼气峰流速(PEF)、最大吸气压(PImax)和用力肺活量(FVC)均高于对照组,差异具有统计学意义(P<0.05);观察组患者血清白细胞介素–8(IL–8)、基质金属蛋白酶–9(MMP–9)、肿瘤坏死因子–α(TNF–α)水平低于对照组,IL–35水平高于对照组,差异具有统计学意义(P<0.05);观察组患者呼吸困难Borg评分低于对照组,6 min步行试验(6MWT)高于对照组,差异具有统计学意义(P<0.05);观察组患者癌因性疲乏情况评分均低于对照组(P<0.05)。观察组患者并发症总发生率明显低于对照组,差异具有统计学意义(P<0.05)。结论:主动循环呼吸训练联合渐进式肌肉放松训练在肺癌康复期有较好的应用效果。Objective To investigate the effect of active circulatory breathing training combined with progressive muscle relaxation training in the rehabilitation period of lung cancer.Methods Eighty cases of patients with lung cancer admitted to Yellow River Sanmenxia Hospital from January 2022 to December 2023 were randomly divided into two groups by drawing lots with 40 cases in each group.The control group received progressive muscle relaxation training,while the observation group received active circulatory breathing training combined with progressive muscle relaxation training.The lung function,inflammatory factors,mobility, cancer-related fatigue, and complications between two groups were compared. Results After intervention, the maximum expiratory pressure (PEmax), forced vital capacity (FVC), peak expiratory flow rate (PEF) and maximum inspiratory pressure (PImax) of the observation group were higher than those of the control group (P < 0.05). After intervention, the levels of serum interleukin-8 (IL-8), matrix metalloproteinase-9 (MMP-9) and tumor necrosis factor-α (TNF-α) in the observation group were lower than those in the control group, while the level of IL-35 was higher than that in the control group (P < 0.05). After intervention, the respiratory distress Borg score of the observation group was lower than that of the control group, and the 6-minute walk test (6MWT) was higher than that of the control group, with statistical significances (P < 0.05). After the intervention, the scores of emotional, severity/behavioral, cognitive/psychological and sensory scores of the observation group were lower than those of the control group (P < 0.05). The total incidence of complications of the observation group was significantly lower than that of the control group (P < 0.05). Conclusion The combination of active circulatory breathing training and progressive muscle relaxation training has a good application effect in the rehabilitation period of lung cancer and is worth promoting.
