临时性支抗装置正畸治疗错[牙合]的临床疗效观察

摘　　要：目的探讨临时性支抗装置(TAD)在错[牙合]畸形矫治中的应用效果及患者生活质量(QOL)的影响。方法选取2017年1月至2023年12月在黔东南苗族侗族自治州人民医院进行矫治的错[牙合]畸形患者100例作为研究对象,通过随机抽样的方法将患者分为试验组和对照组,每组50例。试验组采用TAD支抗进行矫治,对照组采用传统支抗进行矫治。每3周对2组患者的牵引间隙进行临床评估,并在治疗6个月后进行患者QOL评估。结果试验组上颌切牙根尖内收量明显大于对照组,并且试验组的上颌中切牙显著压低,上颌尖牙牙尖的内收速度明显快于对照组,比较差异均有统计学意义(P<0.05)。治疗6个月后,试验组QOL评分显著高于对照组(P<0.05)。结论TAD支抗在错[牙合]畸形治疗中表现出较优的临床效果,可整体内收上颌前牙,以改善牙齿排列和咬合关系,并提高患者的QOL。Objective This study aimed to investigate the application of temporary anchorage devices(TADs)in malocclusion and to compare the effectiveness of using TADs and conventional anchorage therapy in outcomes and patient quality of life(QOL).Methods One hundred cases of malocclusion patients who underwent orthodontic treatment in the People's Hospital of Qiandongnan Miao and Dong Autonomous Prefecture from January 2017 to December 2023 were selected as the study subjects.The patients were divided into an experimental group and a control group by a random sampling method.Patients in the control group were treated with conventional anchorage,and those in the experimental group were treated with TAD anchorage therapy.The space closure of patients in both groups was clinically assessed every 3 weeks,and the QOL of patients was evaluated after 6 months of treatment.Results The maxillary anterior teeth apical retraction in the experimental group was significantly greater than the control group.In the experimental group,maxillary canines were significantly intruded,and the retraction speed was significantly faster(P<0.05).After 6 months of treatment,the QOL scores in the experimental group were significantly higher than the control group(P<0.05).Conclusion TAD anchorage treatment demonstrated superior clinical efficacy for malocclusion.It could be used to achieve overall retraction of maxillary anterior teeth to improve dental alignment and occlusal relationships,and it also improved the QOL of patients.
