摘 要:目的探究联用甜梦口服液对利培酮所致糖脂代谢及内分泌紊乱的作用与机制。方法选取2022—2023年某精神科三甲医院收治的女性精神分裂症住院患者临床资料共112份,分析利培酮单用以及利培酮与甜梦口服液联用后糖脂代谢、甲状腺功能以及性激素指标的变化,并通过动物实验探究甜梦口服液的作用机制。结果与利培酮单用组相比,利培酮与甜梦口服液联用组患者三酰甘油(TG)、总胆固醇(TC)、空腹血糖(FBG)及高密度脂蛋白(HDL-C)水平变化差异无统计学意义,但促甲状腺激素(TSH)、三碘甲腺原氨酸(T3)、甲状腺激素(T4)、游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)水平均有显著变化(P<0.05)。此外,联用甜梦口服液还可以降低利培酮用药患者的血清催乳素(PRL)水平(P<0.01)。大鼠实验的结果与临床数据基本一致,大鼠垂体的转录组学结果则表明甜梦口服液对利培酮所致激素代谢紊乱的改善作用可能与抑制垂体信号传递通路的表达有关。结论联用甜梦口服液可以在一定程度上改善利培酮所致的糖脂代谢及内分泌紊乱,这一作用可能与抑制垂体信号传递通路有关。Objective To determine the effect and mechanism of Tianmeng oral liquid on glucose and lipid metabolism and endocrine disorders induced by risperidone.Methods Totally 112 clinical reports of female schizophrenia inpatients admitted to a psychiatric hospital from 2022 to 2023 were selected.The changes in glucose and lipid metabolism,thyroid function,and sex hormone indexes after treatment with risperidone alone and combined with Tianmeng oral liquid were analyzed.The mechanism of action of Tianmeng oral liquid was determined with animal experiments.Results Compared with the risperidone alone group,no significant difference was found in the levels of triglyceride(TG),total cholesterol(TC),fasting blood glucose(FBG),and high-density lipoprotein(HDL-C)in risperidone combined with Tianmeng oral liquid group,but the levels of triiodothyronine(T3),thyroid hormone(T4),free triiodothyronine(FT3)and free thyroxine(FT4)were up-regulated(P<0.05).In addition,combination of Tianmeng oral liquid also reduced the serum prolactin(PRL)level in patients treated with risperidone(P<0.01).Results of rat experiments were consistent with the clinical data.The transcriptome results of rat pituitary showed that the improvement of Tianmeng oral liquid on hormone metabolism disorder induced by risperidone might be related to the inhibition of the expression of pituitary signaling pathways.Conclusion Combination of Tianmeng oral liquid and risperidone can improve glucose and lipid metabolism and endocrine disorders caused by risperidone to a certain extent,which may be related to the inhibition of the pituitary signaling pathways.
