术中冰冻切片病理检查对乳腺肿物的诊断价值

摘　　要：目的:探究术中冰冻切片病理检查对乳腺肿物的诊断价值。方法:选取中山陈星海中西医结合医院2018年11月—2023年7月收治的1171例患有乳腺肿块且接受手术治疗的患者纳入研究。所有患者均接受术中冰冻切片与石蜡切片病理检查,以石蜡切片病理检查结果作为“金标准”,分析冰冻切片病理检查在乳腺肿物中的诊断价值。结果:石蜡切片检查结果显示,1171例患者中阳性患者89例,占比7.60%。冰冻切片检查结果显示,阴性1084例,阳性87例。两组切片诊断结果一致性较好(Kappa=0.963,P<0.001)。以石蜡切片病理诊断为“金标准”,冰冻切片准确度为99.49%(1165/1171),敏感度为95.51%(85/89),特异度为99.82%(1080/1082),阴性预测值为99.63%(1080/1084),阳性预测值97.70%(85/87)。结论:术中冰冻切片病理检查能准确鉴别乳腺肿物良恶性,诊断准确率较高,在乳腺肿物中具有较高诊断价值。Objective:To explore the value of intraoperative frozen section pathological examination in the diagnosis of breast masses.Methods:A total of one thousand one hundred and seventy-one patients with breast mass and surgical treatment admitted to Zhongshan Chenxinghai Hospital of Integrated Chinese and Western Medicine from November 2018 to July 2023 were included in the study.All patients underwent intraoperative pathological examination of frozen section and paraffin section.Using the results of pathological examination of paraffin section as the"gold standard",the diagnostic value of pathological examination of frozen section in breast tumors was analyzed.Results:The results of paraffin section examination showed that 89 of the 1171 patients were positive,accounting for 7.60%;the results of frozen section examination showed that 1,084 cases were negative and 87 were positive;the diagnostic results of the two groups were consistent(Kappa=0.963,P<0.001).Using paraffin sections as the"gold standard"for pathological diagnosis,the accuracy of frozen sections was 99.49%(1165/1171),sensitivity was 95.51%(85/89),specificity was 99.82%(1080/1082),negative predictive value was 99.63%(1080/1084),and positive predictive value was 97.70%(85/87).Conclusion:Intraoperative pathological examination of frozen section can accurately distinguish benign and malignant breast masses,with high diagnostic accuracy,and has high diagnostic value in breast masses.
