肿瘤标志物联合检测诊断胃癌的价值评估

摘　　要：目的:评估癌胚抗原(CEA)、甲胎蛋白(AFP)、糖类抗原19-9(CA19-9)、糖类抗原242(CA242)、糖类抗原72-4(CA72-4)联合检测诊断胃癌的价值。方法:选取2023年2月—2024年2月南华大学衡阳医学院附属南华医院收治的68例胃癌患者设置为胃癌组,选取同期68例良性胃病患者设置为对照组。对比两组上述肿瘤标志物水平,并以临床综合诊断为“金标准”,分析肿瘤标志物联合检测在胃癌中的诊断效能。结果:胃癌组CEA、AFP、CA19-9、CA242及CA72-4水平均高于对照组(P<0.001)。各肿瘤标志物单一检测及联合检测特异性、阳性预测值对比,无统计学差异(P>0.05);联合检测敏感性、准确率、阴性预测值高于各肿瘤标志物单一检测(P<0.05)。结论:CEA、AFP、CA19-9、CA242及CA72-4联合检测在胃癌中的诊断效能较高,可用于胃癌的早期诊断。Objective:To evaluate the diagnostic value of carcinoembryonic antigen(CEA),alpha-fetoprotein(AFP),carbohydrate antigen 19-9(CA19-9),carbohydrate antigen 242(CA242)and carbohydrate antigen 72-4(CA72-4)in the diagnosis of gastric cancer.Methods:A total of sixty-eight patients with gastric cancer admitted to Nanhua Hospital Affiliated to Hengyang Medical College of University of South China from February 2023 to February 2024 were selected as the gastric cancer group,and sixtyeight patients with benign gastric disease during the same period were selected as the control group.The levels of the above tumor markers were compared between the two groups,and the clinical comprehensive diagnosis was taken as the"gold standard"to analyze the diagnostic efficacy of the combined detection of tumor markers in gastric cancer.Results:The levels of CEA,AFP,CA19-9,CA242 and CA72-4 in gastric cancer group were higher than those in control group(P<0.001).There was no significant difference in specificity and positive predictive value between single detection and combined detection of each tumor marker(P>0.05);the sensitivity,accuracy and negative predictive value of combined detection were higher than those of single detection of each tumor marker(P<0.05).Conclusion:The combined detection of CEA,AFP,CA19-9,CA242 and CA72-4 has high diagnostic efficiency in gastric cancer and can be used for early diagnosis of gastric cancer.
