国产单孔机器人手术系统在儿童卵巢肿瘤切除术中的初步应用

摘　　要：目的:评估国产(术锐)单孔蛇形臂机器人手术系统首次应用于儿童卵巢肿瘤切除术的安全性及有效性。方法:收集2023年1月至11月收治于上海儿童医学中心普外科并接受国产(术锐)单孔手术机器人辅助腹腔镜治疗的13例卵巢肿瘤患儿的临床资料,分析该13例患儿围术期相关临床数据(身高、体质量、手术时间、术中出血量、住院时间等)及随访结果。结果:13例患儿,中位年龄为12(7~16)岁,手术中位时间为105(95~130)min。所有手术均在该机器人手术系统辅助下安全顺利完成,无中转普通腹腔镜或中转开腹病例,术中出血量少且无明显副损伤,术后住院中位时间2d,所有病例均未出现手术相关并发症。术后病理结果:成熟畸胎瘤10例,皮样囊肿1例,浆液性囊肿1例,滤泡囊肿1例。所有病例随访5(1~10)个月均未发现复发且恢复良好。结论:国产(术锐)单孔机器人手术系统应用到儿童卵巢肿瘤切除术中,取得了初步的成功,且该系统安全有效。Objective:To evaluate the safety and effectiveness of a domestic single-hole surgical robot system for the first time in the operation of ovarian tumor in children.Methods:Clinical data of 13 children with ovarian tumor who were admitted to Shanghai Children’s Medical Center from January to July 2023 and received domestic single-hole surgical robot-assisted laparoscopic treatment were collected,and their perioperative clinical data(height,weight,operation time,bleeding volume during operation,hospital stay,etc.)were anlyzed and surgical effect evaluated.Results:There were 13 patients,with a median age of 12(7-16)years and a median operation time of 105 minutes.All operations were completed safely and successfully,with no conversion to conventional laparoscopy or conversion to laparotomy,with little intraoperative blood loss and no obvious collateral damage,and the median postoperative hospitalization time was 2 days.No surgery-related complications occurred in all cases.Postoperative pathological results:10 cases of mature teratoma,1 case of dermoid cyst,1 case of serous cyst,1 case of follicular cyst.No case recurred and all recovered well after follow-up for 5(1-10)months.Conclusion:Domestic(SHURUI)single-hole robot-assisted laparoscopic surgery has been applied to the resection of ovarian tumor with initial success and this system is safe and effective.
