摘 要:目的探讨炎症性肠病(inflammatory bowel disease,IBD)患者肠道菌群特点及其与CD8^(+)CD28^(+)/CD8^(+)CD28^(-)T细胞免疫的相关性。方法选取2021年6月至2023年6月山西白求恩医院医院IBD患者83例,根据疾病类型分为溃疡性结肠炎组(UC组、51例)和克罗恩病组(CD组、32例),另选取同期体检健康成人为对照组(45名)。收集3组基本资料和临床资料,比较3组粪便菌群特点、外周血CD8^(+)CD28^(+)、CD8^(+)CD28^(-)T细胞水平和炎症因子水平,采用Pearson相关分析粪便菌群数量与CD8^(+)CD28^(+)和CD8^(+)CD28^(-)T细胞水平的相关性。结果83例IBD患者中男55例、女28例,年龄20~58岁,平均(39.9±9.2)岁;45名对照组中男26例、女19例,平均(39.7±9.3)岁。3组一般资料的比较,差异无统计学意义(P>0.05)。与对照组相比,UC组和CD组肠道菌群中大肠埃希菌、肠杆菌、酵母菌、拟杆菌、肠球菌群数量较高,消化球菌、乳酸杆菌、双歧杆菌、真杆菌菌群数量较低,差异均有统计学意义(P<0.05)。与对照组相比,UC组和CD组CD8^(+)CD28^(+)T细胞水平和CD8^(+)CD28^(+)/CD8^(+)CD28^(-)较低,CD8^(+)CD28^(-)T细胞水平较高,差异均有统计学意义(P<0.05)。CD8^(+)CD28^(+)/CD8^(+)CD28^(-)T与肠杆菌、酵母菌、拟杆菌呈负相关(r=-0.416、-0.344、-0.471,均P<0.05),与乳酸杆菌、双歧杆菌呈正相关(r=0.354、0.481,均P<0.05)。结论IBD患者肠道菌群数量异常改变,且存在CD8^(+)CD28^(+)/CD8^(+)CD28^(-)T细胞免疫失调,体内炎症反应增强。IBD患者的肠道菌群如肠杆菌、酵母菌、拟杆菌、乳酸杆菌、双歧杆菌数量与CD8^(+)CD28^(+)和CD8^(+)CD28^(-)T细胞水平存在相关性。Objective To explore the characteristics of intestinal flora and their correlation with CD8^(+)CD28^(+)/CD8^(+)CD28^(-)T cell immunity in patients with inflammatory bowel disease(IBD).Methods A total of 83 patients with IBD admitted to Shanxi Bethune Hospital from June 2021 to June 2023 were selected,and were divided into ulcerative colitis group(UC group,51 cases)and Crohn's disease group(CD group,32 cases)according to different disease types,and 45 healthy adults undergoing physical examination during the same period were selected as the control group.The basic data and clincial data of the three groups were collected,and the characteristics of fecal flora,levels of CD8^(+)CD28^(+)and CD8^(+)CD28^(-)T cells and inflammatory factors in peripheral blood were compared.Pearson correlation analysis was used to analyze the correlation between the number of fecal flora and levels of CD8^(+)CD28^(+)and CD8^(+)CD28^(-)T cells.Results Among the 83 IBD patients,there were 55 males and 28 females,aged from 20 to 58 years,with an average of(39.9±9.2)years.There were 26 males and 19 females in the 45 persons in the control groups,with an average age of(39.7±9.3)years.The comparisons of general data among the three groups showed no statistical significance(P>0.05).Compared with the control group,the numbers of Escherichia coli,Enterobacter,Yeast,Bacteroides and Enterococcus in UC group and CD group were higher,while the numbers of Digestive cocci,Lactobacillus,Bifidobacterium and Eubacterium were lower,and the differences were statistically significant(P<0.05).Compared with the control group,the levels of CD8^(+)CD28^(+)T cells and CD8^(+)CD28^(+)/CD8^(+)CD28^(-)T were lower,while the levels of CD8^(+)CD28^(-)T cells were higher in UC group and CD group,with statistical significance(P<0.05).The levels of CD8^(+)CD28^(+)/CD8^(+)CD28^(-)T were negatively correlated with the numbers of Enterobacter,Yeast and Bacteroides(r=-0.416,-0.344,-0.471,P<0.05),while positively correlated with Lactobacillus and Bifidobacterium(r=0.354,0.
