血清基质金属蛋白酶9水平与脑白质病变的相关性分析

摘　　要：目的探讨血清基质金属蛋白酶9(MMP⁃9)水平与脑白质病变(WML)发生的相关性。方法纳入WML患者102例与健康对照者56例,分别收集2组年龄、高血压、糖尿病、吸烟饮酒史等临床资料,以及甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL⁃C)、MMP⁃9等生化指标,同时进行头颅磁共振成像(MRI)检查,根据Fazekas量表评分评估WML病变程度,比较各组的临床资料和生化指标,分析WML发生的危险因素及WML病变程度与MMP⁃9水平的相关性。结果WML组与对照组进行比较,性别、吸烟、饮酒、TG等差异无统计学意义(P>0.05),年龄、高血压、糖尿病、MMP⁃9等差异有统计学意义(P<0.05);不同病变程度WML各组间MMP⁃9水平比较差异有统计学意义(P<0.01);Pearson相关性分析发现,WML病变程度评分与MMP⁃9水平呈正相关(r=0.382,P<0.01)。结论血清MMP⁃9水平升高与WML的发生及病变程度存在一定相关性,检测该指标有助于WML的病情评估和监测。Objective To explore the correlation between serum matrix metalloproteinase⁃9(MMP⁃9)and white matter lesions(WML).Methods A total of 102 WML patients and 56 healthy controls were enrolled in this study.Clinical parameters(such as age,hypertension,diabetes mellitus,history of smoking and drinking)and biochemical indices(triacylglycerol[TG],total cholesterol[TC],low density lipoprotein cholesterol[LDL⁃C],and MMP⁃9)were collected and compared between WML patients and healthy controls.Brain magnetic resonance imaging(MRI)was performed for each participant.Fazekas scale was used to evaluate the severity of WML.The risk factors of WML and the correlation between WML and MMP⁃9 were investigated.Results There were no significant differences in the gender,history of smoking and drinking,or TG level between the two group(P>0.05),but there were significant differences in the age,hypertension,diabetes mellitus and MMP⁃9 level between the two groups(P<0.05).There was significant difference in the MMP⁃9 level among patients with different degrees of WML(P<0.01).Pearson correlation analysis showed that the severity of WML was positively correlated with MMP⁃9 level(r=0.382,P<0.01).Conclusion There is a correlation between elevated serum MMP⁃9 and the development and severity of WML.WML can be monitored and evaluated by testing MMP⁃9.
