摘　　要：口咽癌是常见的头颈部恶性肿瘤之一。人乳头状瘤病毒(HPV)相关口咽癌(HPV+OPC)发病率逐年上升,HPV+OPC具有独特的生物学行为,尤其在淋巴道转移方面与非HPV相关口咽癌具有较大差异,具有发生早、转移率高且预后良好的特点。HPV+OPC淋巴道转移机制目前研究认为与血管内皮生长因子、上皮间充质转化、局部微环境改变等有关。但由于伦理以及缺乏相应的动物模型,相关临床以及机制研究仍然较少,有待进一步研究。本文将从HPV+OPC淋巴道转移的发生机制、临床特点等方面做一综述。Oropharyngeal carcinoma is one of the common malignant tumors of the head and neck.The morbidity of HPV-related oropharygeal carcinoma(HPV+OPC)is rising gradually.HPV+OPC exhibits distinct biological characteristics,and shows to have early onset and high metastatic rate in lymphatic metastasis and favorable prognosis compared with non-HPV-related OPC.Current research suggests that the mechanisms of lymphatic metastasis of HPV+OPC are linked to vascular endothelial growth factor,epithelial-mesenchymal transition and alterations in local microenvironment.However,due to ethical factors and lack of corresponding animal models,relevant clinical and mechanism studies are currently limited.This article reviewed the mechanisms and clinical characteristics of lymphatic metastasis of HPV+OPC.
