摘 要:目的探讨着丝粒蛋白A(CENP-A)在膀胱癌患者中的表达和临床意义。方法从高通量基因表达(GEO)数据库、癌症基因组图谱(TCGA)数据库、GEPIA在线数据库及UALCAN数据库中提取膀胱癌相关的RNA测序数据,检测CENP-A基因在膀胱癌组织和癌旁组织中的表达情况。从TCGA数据库中纳入191例临床病理参数完整的膀胱癌病例资料,根据CENP-A表达量将其分为高表达组(95例)和低表达组(96例)。分析CENP-A基因表达水平与膀胱癌患者临床病理参数的相关性以及与膀胱癌患者预后的相关性。结果在GEO数据库中选取GSE13507数据集,得到1112个差异基因,高表达(logFC为正)的数目有281个,低表达(logFC为负)的数目有831个,其中CENP-A及其伴侣分子HJURP均为高表达(P<0.05)。对TCGA数据库中的406例mRNA Seq数据进行差异基因筛选,得到了1736个差异表达基因,高表达(logFC为正)的数目有854个,低表达(logFC为负)的数目有882个,其中CENP-A为高表达。GEPIA、UALCAN数据库分析结果显示,与正常膀胱组织相比,膀胱癌组织中CENP-A mRNA表达水平升高,差异有统计学意义(均P<0.05)。TCGA数据库分析结果显示,膀胱癌组织中的CENP-A表达水平与膀胱癌患者的年龄、性别、肿瘤浸润程度、淋巴结转移、病理分期均无相关性(均P>0.05),而与肿瘤远处转移有相关性(P<0.05)。单因素分析结果显示,年龄、肿瘤浸润程度、淋巴结转移、远处转移、病理分期均能作为膀胱癌潜在的预后因素(均P<0.05)。多因素Cox模回归分析结果显示,年龄是影响膀胱癌患者预后的独立因素(P<0.05)。结论CENP-A在膀胱癌组织中表达上调且与膀胱癌患者的远处转移情况相关,可能成为膀胱癌精准分子诊断的潜在靶基因,在其诊断治疗和预后判断中发挥重要的作用。ObjectiveTo analyze the expression and clinical significance of centromere protein A(CENP-A)in patients with bladder cancer.MethodsThe RNA sequencing data related to bladder cancer were extracted from Gene Expression Omnibus(GEO)database,The Cancer Genome Atlas(TCGA)database,Gene Expression Profrling Interactive Analysis(GEPIA)online database and the University of ALabama at Birmingham Cancer data analysis Portal(UALCAN)database.The expression of the CENP-A gene was detected in bladder cancer tissues and adjacent tissues,and a total of 191 cases of bladder cancer with complete clinicopathological parameters were included in the TCGA database.According to the expression level of CENP-A,they were divided into high expression group(95 cases)and low expression group(96 cases).The correlation between the expression level of the CENP-A gene and clinicopathological parameters,as well as the prognosis of patients with bladder cancer,was analyzed.ResultsThe GSE13507 dataset was selected from the GEO database and analyzed online using the GEO2R tool.A total of 1112 differentially expressed genes were identified,with 281 showing high expression(positive logFC)and 831 showing low expression(negative logFC).Notably,CENP-A and its companion molecule HJURP exhibited significant upregulation(P<0.05).The TCGA database was utilized to screen for differential genes in 406 cases of mRNA Seq,resulting in the identification of 1736 differentially expressed genes.Among these,854 were found to be highly expressed(positive logFC),while 882 were observed to have low expression levels(negative logFC).Notably,CENP-A exhibited high expression levels.Subsequent analysis using the GEPIA and UALCAN databases revealed a statistically significant increase in the expression level of CENP-A mRNA in bladder cancer tissues compared to normal bladder tissues(all P<0.05).The TCGA data analysis revealed a positive correlation was observed with distant tumor metastasis(P<0.05).Univariate analysis indicated that age,degree of tumor invasion,lymph node met
