摘 要:目的探讨膀胱癌组织中miR-493-5p和高尔基体膜蛋白1(GOLM1)的水平变化及其临床意义。方法选取2016年3月至2018年2月在本院就诊的100例膀胱癌患者作为研究对象,收集膀胱癌患者手术切除的癌组织及癌旁组织保存备用。采用实时荧光定量PCR(qRT-PCR)测定膀胱癌组织和癌旁组织中的miR-493-5p、GOLM1 mRNA水平,采用免疫组化染色检测膀胱癌组织和癌旁组织中的GOLM1蛋白表达水平,采用Pearson分析膀胱癌组织中的miR-493-5p水平与GOLM1 mRNA水平的相关性,采用Kaplan-Meier法绘制膀胱癌患者的5年生存曲线,采用Cox回归分析膀胱癌患者的5年生存率的影响因素。结果与癌旁组织相比较,膀胱癌组织的miR-493-5p水平降低,而GOLM1 mRNA水平升高,差异均有统计学意义(均P<0.05)。GOLM1蛋白在膀胱癌组织中的阳性表达率为64.00%(64/100),高于癌旁组织中的阳性率[12.00%(12/100)],差异有统计学意义(P<0.05)。miR-493-5p、GOLM1均与患者淋巴结是否转移、TNM分期、分化等级以及浸润深度均有相关性(均P<0.05)。Pearson相关分析结果显示,膀胱癌组织中miR-493-5p水平与GOLM1 mRNA水平呈负相关(r=-0.517,P<0.05)。miR-493-5p与GOLM1可能存在靶向关系。miR-493-5p高表达、GOLM1阴性患者的5年生存率高于miR-493-5p低表达、GOLM1阳性患者(χ2=5.881、5.556,均P<0.05)。高水平miR-493-5p为膀胱癌患者死亡的保护因素(P<0.05),高水平GOLM1和淋巴结转移为膀胱癌患者死亡的危险因素(P<0.05)。结论膀胱癌组织中miR-493-5p水平降低,GOLM1水平升高,与膀胱癌患者的5年生存率相关,评估膀胱癌患者预后具有一定价值。ObjectiveTo investigate the changes of microRNA-493-5p(miR-493-5p)and Golgi membrane protein 1(GOLM1)levels in bladder cancer tissues and their clinical significance.MethodsA total of 100 patients with bladder cancer hospitalized in our hospital from March 2016 to February 2018 were regarded as the study subjects.The tumor tissue and adjacent tissue of bladder cancer patients were collected.The miR-493-5p and GOLM1 mRNA levels in bladder cancer tissues and adjacent tissues were measured by qRT-PCR.The expression of GOLM1 protein in bladder cancer tissues and adjacent tissues was detected by immunohistochemical staining.Pearson was applied to analyze the correlation between miR-493-5p level and GOLM1 mRNA level in bladder cancer tissues.The 5-year survival curve of bladder cancer patients was drawn by Kaplan-Meier method.Cox regressionwas applied to analyze the influencing factors of 5-year survival rate of bladder cancer patients.ResultsCompared with adjacent tissues,the level of miR-493-5p in bladder cancer tissue was obviously decreased,while the level of GOLM1 mRNA was increased,with statistical significance(all P<0.05).The positive expression rate of GOLM1 protein in bladder cancer tissues was 64.00%(64/100),which was obviously higher than that in adjacent tissues[12.00%(12/100)],and the difference was statistically significant(P<0.05).The levels of miR-493-5p and GOLM1 mRNA were related to lymph node metastasis,TNM stage,differentiation grade and depth of invasion(all P<0.05).Pearson correlation analysis showed that the level of miR-493-5p in bladder cancer tissue was negatively correlated with the level of GOLM1(r=-0.517,P<0.05).miR-493-5p may have a targeting relationship with GOLM1.The 5-year survival rate of patients with high expression of miR-493-5p and GolM1-negative expression was obviously higher than those with low expression of miR-493-5p and GOLM1-positive expression(χ2=5.881,5.556,all P<0.05).High levels of miR-493-5p were protective factors for death of bladder cancer patients(P<0.05),and high
