自拟愈溃汤联合美沙拉嗪肠溶片治疗脾肾阳虚型溃疡性结肠炎的临床效果

摘　　要：目的观察自拟愈溃汤联合美沙拉嗪肠溶片治疗脾肾阳虚型溃疡性结肠炎(UC)的临床效果。方法选取2023年6月—2024年6月九江市中医医院收治的脾肾阳虚型UC患者60例,按照随机盲抽法分为观察组和对照组,每组30例。对照组给予美沙拉嗪肠溶片治疗,观察组在对照组基础上给予自拟愈溃汤治疗,2组均治疗4周。比较2组临床疗效,治疗前后中医证候积分、改良的Mayo活动指数评分、炎症性肠病(IBD)生活质量量表评分。结果观察组总有效率为96.67%,高于对照组的70.00%(χ^(2)=7.680,P=0.006)。治疗4周后,2组腹泻、腹痛、脓血便、里急后重积分及腹泻、出血、黏膜外观、医师评估评分低于治疗前,且观察组低于对照组(P<0.01);2组IBD生活质量量表评分高于治疗前,且观察组高于对照组(P<0.01)。结论自拟愈溃汤联合美沙拉嗪肠溶片治疗脾肾阳虚型UC的临床效果较好,可有效改善患者临床症状。Objective To observe the clinical effect of self-prepared Yukui decoction combined with mesalazine enteric-coated tablets in treating ulcerative colitis(UC)of Pishen Yangxu type.Methods Sixty patients with UC of Pishen Yangxu type admitted to Jiujiang Hospital of Traditional Chinese Medicine from June 2023 to June 2024 were selected and divided into observation group and control group according to random blind sampling method,with 30 cases in each group.The control group was treated with mesalazine enteric-coated tablets,and the observation group was treated with self-prepared Yukui decoction on the basis of the control group.Both groups were treated for 4 weeks.The clinical efficacy,TCM syndrome score,improved Mayo activity index score and IBD quality of life scale score before and after treatment were compared between the two groups.Results The total effective rate of the observation group was 96.67%,which was higher than that of the control group(70.00%)(χ^(2)=7.680,P=0.006).After 4 weeks of treatment,the TCM syndrome scores of both groups,including diarrhea,abdominal pain,bloody mucus stool and tenesmus,as well as the Mayo activity index scores,which include diarrhea,bleeding,mucosal appearance and physician′s assessment,were lower than those before treatment.Moreover,the observation group showed a significantly lower score compared to the control group(P<0.01).The scores of IBD quality of life scale in 2 groups were higher than before treatment,and those in observation group were higher than those in control group(P<0.01).Conclusion The clinical effect of self-prepared Yukui decoction combined with mesalazine enteric-coated tablets in the treatment of UC of Pishen Yangxu type is better,which can effectively improve the clinical symptoms of patients.
