血清TG及改良早期预警评分对高脂血症胰腺炎患者预后的预测价值

摘　　要：目的:探究血清甘油三酯(TG)及改良早期预警评分(MEWS)对高脂血症胰腺炎患者预后的预测价值。方法:选取2020年1月—2023年12月新余市人民医院收治的102例高脂血症胰腺炎患者作为研究对象,收集患者一般资料、血清TG检测值、MEWS,根据患者30 d后生存情况分为预后良好组(n=70)、预后不良组(n=32)。对两组进行单因素分析与多因素logistic回归分析,分析影响患者预后的危险因素,绘制受试者操作特征(ROC)曲线分析血清TG及MEWS对高脂血症胰腺炎患者预后的预测价值。结果:单因素分析结果显示,两组性别、年龄、体重指数(BMI)比较,差异均无统计学意义(P>0.05);预后良好组TG水平、MEWS均低于预后不良组,差异均有统计学意义(P<0.05)。多因素分析结果显示,TG、MEWS是高脂血症胰腺炎患者预后不良的危险因素(P<0.05)。ROC曲线显示,TG、MEWS联合预测的AUC、敏感度、特异度均高于TG、MEWS单独预测(P<0.05)。结论:血清TG及MEWS联合预测高脂血症胰腺炎患者预后的价值较高。Objective:To explore the prognostic value and feasibility of serum triglyceride(TG)and modified early warning score(MEWS)in patients with hyperlipidemia pancreatitis.Method:A total of 102 patients with hyperlipidemia pancreatitis admitted to Xinyu People's Hospital from January 2020 to December 2023 were selected as the study objects.General data,serum TG detection values and MEWS were collected.Patients were divided into good prognosis group(n=70)and poor prognosis group(n=32)according to their survival after 30 days.Univariate analysis and multivariate logistic regression analysis were performed to analyze the risk factors affecting the prognosis of the patients,and receiver operating characteristics(ROC)curve was drawn to analyze the predictive value of serum TG and MEWS in the prognosis of patients with hyperlipidemia pancreatitis.Result:Univariate analysis showed that there were no significant differences in gender,age and body mass index(BMI)between the two groups(P>0.05).TG level and MEWS in the good prognosis group were lower than those in the poor prognosis group,the differences were statistically significant(P<0.05).The results of multi-factor analysis showed that TG and MEWS were risk factors for poor prognosis in patients with hyperlipidemia pancreatitis(P<0.05).ROC curve showed that AUC,sensitivity and specificity of TG and MEWS combined prediction were higher than those of TG and MEWS alone prediction(P<0.05).Conclusion:The combined value of serum TG and MEWS in predicting the prognosis of patients with hyperlipidemia pancreatitis is higher.
