高危型HPV感染宫颈癌血清miR18a-和miR34a-5-p与miR195-的诊断价值

摘　　要：目的探讨高危型人乳头瘤病毒(HPV)感染宫颈癌血清微小核糖核酸(miR)-18a和miR-34a-5p与miR-195的诊断价值。方法选取2022年1月-2023年7月河南大学淮河医院的高危型HPV感染宫颈癌患者77例为研究组,同期高危型HPV阳性宫颈鳞状上皮内病变患者79例为对照组。比较两组miR-18a、miR-34a-5p、miR-195水平;比较研究组不同特征患者miR-18a、miR-34a-5p、miR-195水平,分析三指标预测价值。结果研究组血清miR-18a水平较对照组高(P<0.05),miR-34a-5p、miR-195水平较对照组低(P<0.05)。研究组有淋巴结转移患者miR-34a-5p、miR-195水平较无淋巴结转移低(P<0.05),miR-18a水平比无淋巴结转移高(P<0.05),病理分期Ⅲ期患者miR-18a比病理分期Ⅰ~Ⅱ期高(P<0.05)。三指标联合检测诊断高危型HPV感染宫颈癌的曲线下面积(AUC)比单独检测高(P<0.05),敏感度和特异度为81.80%和83.50%。结论高危型HPV感染宫颈癌患者血清miR-18a呈高表达,miR-34a-5p、miR-195呈低表达,联合检测有助于诊断高危型HPV感染宫颈癌,三指标与HPV阳性宫颈癌患者不同特征有关。OBJECTIVE To explore the values of serum microribonucleic acid(miR)-18a,miR-34a-5p and miR-195 in diagnosis of high-risk human papillomavirus(HPV)infection in the cervical cancer patients.METHODS Totally 77 cervical cancer patients with high-risk HPV infection who were treated in Huaihe Hospital,Henan University from Jan.2022 to Jul.2023 were assigned as the study group.Meanwhile,79 high-risk HPV-positive patients who had cervical squamous intraepithelial lesions were chosen as the control group.The levels of miR-18a,miR-34a-5p amd miR-195 were compared between the two groups.The levels of miR-18a,miR-34a-5p and miR-195 were compared between the IL-17 methylation-positive patients and the IL-17 methylation-negative patients.The predictive values of the three indexes were analyzed,and the short-term curative effect of the different state of IL-17 methylation was observed.RESULTS The serum miR-18a level of the study group was higher than that of the control group(P<0.05),while the levels of miR-34a-5p and miR-195 of the study group was lower than those of the control group(P<0.05).In the study group,the levels of miR-34a-5p and miR-195 of the patients with lymphatic metastasis were lower than those of the patients without lymphatic metastasis(P<0.05),while the miR-18a level of the patients with lymphatic metastasis was higher than that of the patients without lymphatic metastasis(P<0.05);the miR-18a level of the patients with stageⅢof pathological stage was higher than that of the patients with the pathological stage ranging betweenⅠandⅡ(P<0.05).The area under curve(AUC)of the joint detection of the three indexes was higher than that of the single detection in diagnosis of the cervical cancer patients with high-risk HPV infection(P<0.05),the sensitivity was 81.80%,with the specificity 83.50%.CONCLUSIONS The cervical cancer patient with high-risk HPV infection show high expression of serum miR-18a and the low expressions of miR-34a-5p and miR-195.The joint detection of the indexes may facilitate the diagnosis
