摘　　要：目的探究乙型肝炎病毒(HBV)感染相关肝硬化血清转化生长因子-β1(TGF-β1)、高尔基体蛋白-73(GP-73)、嗜酸性粒细胞趋化因子(CCL11)、Toll样受体4(TLR4)水平及其与Child-Pugh分级的相关性。方法选取中国医科大学附属盛京医院2020年10月-2023年10月收治的HBV感染相关肝硬化患者192例为研究组,根据Child-Pugh分级将研究组分为A级76例、B级62例和C级54例;选取同期健康体检者201名为对照组。比较研究组、对照组之间及A、B、C级患者之间血清TGF-β1、GP-73、CCL11、TLR4水平,采用Spearman分析血清TGF-β1、GP-73、CCL11、TLR4水平与HBV感染相关肝硬化患者Child-Pugh分级的相关性,采用受试者工作特征(ROC)曲线分析四指标对HBV感染相关肝硬化患者的诊断价值。结果研究组血清TGF-β1、GP-73、CCL11、TLR4水平高于对照组(P<0.05)。C级组、B级组血清TGF-β1、GP-73、CCL11、TLR4水平高于A级组(P<0.05),且C级组高于B级组(P<0.05)。血清TGF-β1、GP-73、CCL11、TLR4水平与HBV感染相关肝硬化患者Child-Pugh分级呈正相关(P<0.05)。四指标联合检测诊断HBV感染相关肝硬化患者的曲线下面积(AUC)值高于各指标单一检测(P<0.05)。结论血清TGF-β1、GP-73、CCL11、TLR4水平在HBV感染相关肝硬化患者中表达增加,四者水平与患者Child-Pugh分级呈正相关,且四者联合检测对其诊断价值高。OBJECTIVE To explore the levels of serum transforming growth factor-β1(TGF-β1),Golgi protein-73(GP-73),eosinophilic chemotactic factor(CCL11)and Toll-like receptor 4(TLR4)of the patients with hepatitis B virus(HBV)infection-associated liver cirrhosis and analyze the association with Child-Pugh grade.METHODS A total of 192 patients with HBV infection-associated liver cirrhosis who were treated in Shengjing Hospital of Chine Medical University from Oct.2020 to Oct.2023 were assigned as the study group and were divided into the patients of grade A with 76 cases,the patients of grade B with 62 cases and the patients of grade C with 54 cases according to the Child-Pugh grade.Meanwhile,201 healthy people who received physical examination were chosen as the control group.The levels of serum TGF-β1,GP-73,CCL11 and TLR4 were compared between the study group and the control group as well as among the grade A,grade B and grade C patients.Spearman analysis was performed for the association between the levels of serum TGF-β1,GP-73,CCL11 and TLR4 and the Child-Pugh grade of the patients with HBV infection-associated liver cirrhosis.The values of the four indexes in diagnosis of HBV infection-associated liver cirrhosis were analyzed by means of receiver operating characteristic(ROC)curves.RESULTS The levels of serum TGF-β1,GP-73,CCL11 and TLR4 of the study group were significantly higher than those of the control group(P<0.05).The levels of serum TGF-β1,GP-73,CCL11 and TLR4 of the grade C group and the grade B group were significantly higher than those of the grade A group(P<0.05),and the levels of the above indexes of the grade C group were significantly higher than those of the grade B group.There was positive association between the levels of serum TGF-β1,GP-73,CCL11 and TLR4 and the Child-Pugh grade of the patients with HBV infection-associated liver cirrhosis(P<0.05).The area under the curve(AUC)of the joint detection of the four indexes was significantly higher than that of the single detection in diagnosis of the
