腹腔镜D3根治术中保留左结肠动脉对老年直肠癌患者术后肛门功能的影响

摘　　要：目的探讨腹腔镜D3根治术中保留左结肠动脉对老年直肠癌患者术后肛门功能的影响。方法选取医院2021年3月至2023年5月收治的老年直肠癌患者112例,按收治时间先后顺序分为观察组和对照组,各56例。两组患者均行腹腔镜D3根治术治疗,观察组患者术中保留左结肠动脉,对照组患者术中不保留左结肠动脉。结果与对照组比较,观察组患者的手术总时间显著更长,但术后的肛门首次排气时间及住院总时间均显著更短(P<0.05)。观察组患者术后的肛管最大收缩压、肛管静息压均显著高于对照组,低位前切除综合征评分显著低于对照组(P<0.05)。结论腹腔镜D3根治术中保留左结肠动脉可缩短老年直肠癌患者的肛门首次排气时间及住院总时间,升高肛管最大收缩压、肛管静息压,降低低位前切除综合征评分。Objective To investigate the effect of preserving left colonic artery in laparoscopic D3 radical surgery on postoperative anal function of elderly rectal cancer patients.Methods A total of 112 elderly rectal cancer patients admitted to the hospital from March 2021 to May 2023 were selected and divided into observation group and control group according to the order of the admission time,with 56 cases in each group.Both groups were treated with laparoscopic D3 radical surgery,and the left colonic artery was preserved in the observation group,while the left colonic artery was not preserved in the control group.Results Compared with that in the control group,the total operation time was significantly longer in the observation group,but the time to postoperative first anal evacuation and the total hospitalization time was significantly shorter(P<0.05).The postoperative anal canal maximal systolic pressure and anal canal resting pressure of the patients in the observation group were significantly higher than those in the control group,and the low anterior resection syndrome score was significantly lower than that in the control group(P<0.05).Conclusion Preservation of the left colonic artery during laparoscopic D3 radical surgery can shorten the time of first anal exsufflation and the total hospitalization time of elderly patients with rectal cancer,increase the maximal systolic pressure and resting pressure of the anal canal,and reduce the score of low anterior resection syndrome.
