固定义齿修复与活动义齿修复在牙周病伴牙列缺失患者中的对比研究

摘　　要：目的:探讨活动义齿修复与固定义齿修复对牙周病伴牙列缺失患者牙周健康及牙齿功能的影响。方法:选取2021年1月—2023年12月在无锡市第二人民医院接受治疗的牙周病伴牙列缺失患者80例,按随机数字表法分为两组,A组(40例,44颗牙)采用活动义齿修复方案,B组(40例,46颗牙)采用固定义齿修复方案,随访3个月。评估并对比两组临床疗效、牙周健康(牙龈指数、探诊出血指数、牙周袋深度)、牙齿功能(咬合力及咀嚼效率)和生活质量(口腔健康影响程度量表评分)。结果:A组有2例失访,共38例(42颗牙)纳入分析,B组无失访,共40例(46颗牙)纳入分析。随访3个月后,B组总有效率为97.50%(39/40),A组为81.58%(31/38),两组比较差异无统计学意义(P>0.05)。随访3个月后,两组牙周健康指标均较修复前改善,B组牙龈指数、探诊出血指数、牙周袋深度均低于A组,差异均有统计学意义(P<0.05)。随访3个月后,两组牙齿功能指标均较修复前提高,B组咬合力、咀嚼效率均高于A组,差异均有统计学意义(P<0.05)。随访3个月后,A组口腔健康影响程度量表得分低于B组,差异有统计学意义(P<0.05)。结论:固定义齿修复对牙周病伴牙列缺失患者的牙齿功能和牙周健康有更好的改善效果,活动义齿修复在短期生活质量改善上更具优势,建议在临床上根据患者的具体状况选择适合的修复方案。Objective:To explore the effects of removable denture restoration and fixed denture restoration on periodontal health and tooth function in patients with periodontitis and missing teeth.Method:Eighty patients with periodontitis and missing teeth treated in Wuxi Second People's Hospital from January 2021 to December 2023 were selected and divided into two groups according to random number table method.Group A(40 patients,44 teeth)received removable denture restoration,while group B(40 patients,46 teeth)received fixed denture restoration.Follow-up was conducted for three months.The clinical efficacy,periodontal health(gingival index,probing bleeding index,periodontal pocket depth),tooth function(bite force and masticatory efficiency),and quality of life(oral health impact profile)were evaluated and compared between the two groups.Result:Two patients in group A were lost to follow-up,and a total of 38 patients(42 teeth)were included in the analysis.Group B had no missing visits,with a total of 40 cases(46 teeth)included in the analysis.After 3 months of follow-up,the total effective rate was 97.50%(39/40)in group B and 81.58%(31/38)in group A,there was no significant difference between the two groups(P>0.05).After 3 months of follow-up,the periodontal health indexes of both groups were improved compared with those before restoration,and the gingival index,probing bleeding index and periodontal pocket depth of group B were lower than those of group A,with statistical significance(P<0.05).After 3 months of follow-up,the dental function indexes of both groups were improved compared with those before restoration,and the biting force and masticatory efficiency of group B were higher than those of group A,with statistical significance(P<0.05).After 3 months of follow-up,the score of oral health impact scale in group A was lower than that in group B,with statistical significance(P<0.05).Conclusion:Fixed denture restoration provides better improvement in tooth function and periodontal health in patients with periodontitis a
